摘 要:目的:研究临床分离的多药耐药肠杆菌科细菌产超广谱β‐内酰胺酶(ESBLs)基因型,为临床治疗产ES‐BLs的肠杆菌科细菌提供依据。方法收集2011年5月-2012年5月临床分离的27株肠杆菌科细菌,运用VITEK‐2药敏试验复合板进行药敏试验,分析其耐药性,利用聚合酶链反应(PCR)技术扩增 ESBLs 基因blaCTX、blaTEM和blaSHV ;测序结果与GenBank数据库进行比对分析,以确定ESBLs的主要基因亚型。结果27株肠杆菌科细菌均呈现多药耐药性,其对头孢菌素类、青霉素类和碳青霉烯类的耐药率为40.7%-100.0%;PCR扩增出18株细菌携带2种以上ESBLs基因,6株携带1种ESBLs基因,有3株细菌未检测到ESBLs基因;ESBLs基因亚型主要为 blaCTX‐M‐3、blaTEM‐1和blaSHV‐12。结论医院多药耐药的肠杆菌科细菌产ESBLs基因较普遍,其主要亚型为 blaCTX‐M‐3、blaTEM‐1和blaSHV‐12。OBJECTIVE To investigate the genotypes of extended‐spectrumβ‐lactamases (ESBLs) produced by clin‐ical isolates of multidrug‐resistant Enterobacteriaceae so as to provide guidance for clinical treatment of the ESBLs‐producing Enterobacteriaceae .METHODS Totally 27 clinical Enterobacteriaceae isolates were included in the stud‐y ,then the drug susceptibility testing was performed by using VITEK‐2 Composite panel for drug susceptibility testing ,and the drug resistance was analyzed .The ESBLs genes such as blaCTX ,blaTEM ,and blaSHV were amplified by using polymerase‐chain‐reaction (PCR) technique ,the sequencing results were compared with the GenBank database so as to determine the major genotypes of the ESBLs .RESULTS All the 27 strains of Enterobac‐teriaceae were multidrug‐resistant ,and the drug resistance rate to cephalosporins ,penicillins ,and carbapenems ranged from 40 .7% to 100 .0% .The PCR amplification showed that 18 strains carried with more than two ESBLs genes ,6 strains carried with one ESBLs gene ,and 3 strains were detected negative for the ESBLs genes ;the blaCTX‐M‐3 ,blaTEM‐1 ,and blaSHV‐12 were the major genotypes of ESBLs .CONCLUSION The ESBLs genes are prevalent in the multidrug‐resistant Enterobacteriaceae strains , and the blaCTX‐M‐3 , blaTEM‐1 , and blaSHV‐12 are the major genotypes .
