摘 要:目的:了解腹部Ⅲ类手术切口感染的危险因素,以便采取有效的预防措施,降低切口感染率。方法回顾性分析2010年8月-2013年6月急诊外科204例腹部Ⅲ类手术切口患者临床资料,每份病例均填写统一调查表格,所有数据采用SPSS 11.5软件进行统计处理。结果2010年8月-2013年6月204例患者中切口感染48例,感染率23.5%;单因素分析显示,切口感染率≥60岁、肥胖、切口长度≥12 cm、手术时间≥2 h及有基础疾病均高于14-59岁,非肥胖、切口长度<12 cm及无基础疾病患者(P<0.05);logistic多因素回归分析显示,高龄、肥胖、手术切口长度、手术时间、基础疾病是腹部Ⅲ类手术切口感染的独立危险因素。结论腹部Ⅲ类手术切口高感染率是多因素作用的结果,临床上应采取综合措施进行相应的干预以降低感染率。OBJECTIVE To observe the risk factors for incision infections in patients undergoing type Ⅲ abdominal surgery and take effective prevention measures so as to prevent the incision infections .METHODS The clinical data of 204 patients who underwent the type Ⅲ abdominal surgery in emergency surgery department from Aug 2010 to Jun 2013 were retrospectively analyzed ,the unified questionnaire survey tables were filled out by each case ,and all the data were statistically analyzed by using SPSS 11 .5 software .RESULTS From Aug 2010 to Jun 2013 ,the inci‐sion infections occurred in 48 of 204 patients ,with the infection rate of 23 .5% .The univariate analysis indicated that the incidence of incision infections was higher in patients no less than 60 years old ,obesity ,length of incision no less than 12 cm ,operation duration no less than 2 hours ,or underlying diseases than in patients aged between 14 and 59 years old ,without obesity ,length of incision less than 12cm or without underlying diseases (P〈0 .05) . The multivariate logistic regression analysis indicated that the advanced age ,obesity ,length of surgical incision , operation duration ,and underlying disease were the independent risk factors for the incision infections in the pa‐tients undergoing type Ⅲ abdominal surgery .CONCLUSION There are a variety of risk factors to result in the high incidence of incision infections in the patients undergoing type Ⅲ abdominal surgery ,and it is necessary for the hospital to take targeted interventions to reduce the infection rate .
