摘 要:目的探讨非典型肺炎病原体免疫球蛋白(IgM)检测对儿童下呼吸道感染的诊断意义,了解下呼吸道感染患儿病原谱及病原学流行特点,为临床诊治提供参考。方法用间接免疫荧光法检测2012年11月-2013年10月2 495例下呼吸道感染患儿血清中肺炎支原体、乙型流感病毒、嗜肺军团菌血清Ⅰ型、腺病毒、呼吸道合胞病毒、甲型流感病毒、肺炎衣原体、副流感病毒、Q热立克次体等9种病原体IgM,并对以上病原体流行特点进行分析。结果 2 495份标本中共有1 132份检出病原体IgM,阳性检出率为45.4%,检出率居前3位的依次为肺炎支原体、乙型流感病毒及嗜肺军团菌血清Ⅰ型,阳性检出率分别为36.7%、10.7%、6.5%,肺炎支原体占绝对优势;在IgM阳性病例中,35.8%的患儿发生≥2种病原体混合感染,肺炎支原体、乙型流感病毒、嗜肺军团菌血清Ⅰ型及副流感病毒Ⅰ型等的感染具有明显的季节性差异,肺炎支原体及副流感病毒Ⅰ型感染以夏季为著,嗜肺军团菌血清Ⅰ型感染以冬季与夏季为著,而乙型流感病毒感染则以冬季与春季为著。结论肺炎支原体是儿童下呼吸道感染的主要病原体,冬季与夏季是儿童更容易发生感染的季节。OBJECTIVE To explore the clinical significance of detection of immunoglobulin of pathogens causing a‐typical pneumonia in diagnosis of lower respiratory tract infections in children and understand the pathogenic spec‐trum and etiological characteristics of the children with lower respiratory tract infections in Guilin area so as to provide guidance for clinical diagnosis and treatment .METHODS A total of 2 495 children with lower respiratory tract infections who were treated in the hospital from Nov 2012 to Oct 2013 were enrolled in the study ,then the immunoglobulins of 9 species of pathogens isolated from serum of the participants ,including the Mycoplasma pneumoniae ,influenza B virus ,Legionella pneumophila serotype Ⅰ ,adenovirus ,respiratory syncytial virus ,in‐fluenza A virus ,Chlamydia pneumoniae ,parainfluenza virus ,and rickettsia burneti ,were detected by using indi‐rect immunofluorescence assay ;the epidemiological characteristics of the pathogens were observed .RESULTS Of 2 495 specimens ,1 132 were tested positive for immunoglobulins of the pathogens ,with the total positive detection rate of 45 .4% ;the M .pneumoniae ,influenza B virus ,and L .pneumophila serotype Ⅰ ranked the top three spe‐cies of pathogens ,with the positive detection rates of 36 .7% ,10 .7% ,and 6 .5% ,respectively ,and the M .pneu‐moniae was dominant .Among the children tested positive for immunoglobulins ,35 .8% were infected with no less than 2 species of pathogens ;the infection of M .pneumoniae ,influenza B virus ,L .pneumophila serotype Ⅰ ,or parainfluenza virus type Ⅰ varied significantly in the different seasons ;the infection of M .pneumoniae or parain‐fluenza virus type Ⅰ was most prevalent in the summer ,the L .pneumophila serotype Ⅰ infection was the most common in the winter and summer ,however ,the influenza B virus infection was the most prevalent in the winter and spring .CONCLUSION The M .pneumoniae is the predominant species of pathogen causing lower respiratory tract infections in the
