摘 要:目的探讨2型糖尿病(T 2DM)合并非酒精性脂肪肝(NAFLD)的危险因素。方法选取住院T 2DM患者104例,根据肝脏B超检查结果分为NAFLD组(n=56)和非NAFLD组(n=48),回顾性分析2组患者的各个相关临床指标,采用Logistic多元回归分析筛选危险因素。结果 NAFLD组的体重指数(BMI)、空腹血清葡萄糖(FPG)、血清胰岛素(FIn S)、胰岛素抵抗指数(HOMA-IR)、血清总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白(LDL-C)、血尿酸水平(UA)均显著高于非NAFLD组,高密度脂蛋白(HDL-C)显著低于非NAFLD组,上述差异均有统计学意义(P<0.05)。多元回归分析表明,BMI、TG是预测糖尿病患者发生NAFLD的独立危险因素(P<0.05)。结论 T 2DM合并NAFLD与肥胖、脂代谢紊乱、胰岛素抵抗密切相关。Objective To explore the risk factors of type 2 diabetic patients (T 2DM) complicated with non-alcoholic fatty liver disease (NAFLD). Methods 104 patients with T 2DM were divided into NAFLD group and non-NAFLD group according to the results of ultrasonic images. Clinical indicators were retrospectively analyzed in the two groups. Logistic multiple regression analysis was used to analyze the risk factors. Results The levels of body mass index(BMI), fasting plasma glucose(FPG), serum insulin(FInS),homeostatic model assessment of insulin resistance(HOMA-IR), serum total cholesterol(TC), triglycerides(TG), low density lipoprotein cholesterol(LDL-C) and uric acid(UA) in NAFLD group were all significantly higher than that in non-NAFLD group(P〈0.05). The level of high-density lipoprotein cholesterol(HDL-C) in NAFLD group was significantly lower than that in non-NAFLD group(P〈0.05). Logistic regression analysis showed that TG and BMI were independent risk factors for NAFLD (P〈0.05). Conclusion T 2DM complicated with NAFLD may have a close association with obesity, lipid metabolic disorder and insulin resistance.
