化学法、单克隆抗体法和联合免疫法检测便隐血的临床比较

摘  要:目的对比化学法、单克隆抗体法和联合免疫法检测便隐血的临床效果。方法选取大便标本90例,采用化学法、单克隆抗体法和联合免疫法3种试验方法,对其进行便隐血检测。结果化学法、单克隆抗体法和联合免疫法3种方式检测出血红蛋白的最低检出率分别为2、0.2、0.2 mg/L;化学法对人和动物的血红蛋白可发生反应,联合免疫法和单克隆抗体仅和人的血红蛋白发生反应;维生素C浓度超过10 g/L可引起化学法的阴性结果,而对联合免疫法和单克隆抗体的检测无影响。化学法、单克隆抗体法和联合免疫法3种不同的检测方法检测上消化道出血组的阳性率分别为95.2%、72.1%、91.2%,检测下消化道出血组的阳性率分别为75.6%、96.8%、100%;检测正常组的阳性率分别为10.5%、3.2%、6.8%。结论化学法、单克隆抗体法和联合免疫法3种检测方法相互结合、相互补充,有助于提高消化道出血性疾病的检出率,为临床提供更加精确的结果。Objective To compare the therapeutic effect of chemical method, monoclonal antibody and combined immunoassay for fecal occult blood. Methods 90 cases of fecal specimens were selected admitted in our hospital from May 2013 to May 2014 , using chemical method, monoclonal antibody and combined immunization of three test methods of fecal occult blood testing on it. Results The chemical method, monoclonal antibody method combined with immune method in three ways to detect hemoglobin detection rates were 2, 0.2, 0.2 mg/L;hemoglobin chemical method for human and animal could react, combined immunoassay and monoclonal antibody alone and human hemoglobin reacts with negative results;vitamin C concentrations exceeding 10 g/L can be caused by chemical method, without influence on the joint detection of immunological method and monoclonal antibodies. The detection of three different detection methods of upper digestive tract hemorrhage group the positive rate were 95.2%, 72.1%, 91.2%, three different methods for the detection of lower digestive tract hemorrhage group the positive rate were 75.6%, 96.8%, 100%;the positive detection rate of normal group of three different detection methods were 10.5%, 3.2%, 6.8%. Conclusion The three methods for detection of chemical method, monoclonal antibody and combined immunization with each other, complement each other, help to improve the detection rate of diseases of digestive tract hemorrhage, provide more accurate results for clinical, worthy of further clinical application.
