摘 要:目的观察慢性心力衰竭患者营养状况,并探讨其与甲状腺功激素水平变化的关系。方法依据纽约心脏病学会分级,对48例心功能Ⅲ~Ⅳ级的慢性心力衰竭患者进行营养状况评估,并分组:营养正常组28例,营养不良组20例。比较两组总三碘甲状腺原氨酸(TT3),血清总甲状腺素(TT4)和促甲状腺激素(TSH)水平。结果营养不良发生率41.7%(20/48)。与正常组比较,营养不良组血清TT3和TT4降低,TSH上升,TT3的差异具有统计学意义(P〈0.01)。结论慢性心力衰竭患者营养不良的发生率较高,且营养不良发生与甲状腺功能的改变之间可能存在一定联系。Objective To observe the nutrition status and changes of thyroid function in patients with chronic heart failure (CHF). Methods Forty-eight CHF patients classified as NYHA Ⅲ orⅣ underwent nutrition evaluation were divided into normal nutrition group (n= 28) and malnutrition group (n= 20). The changes of thyroid hormone levels including total triiodothyronine (TT3), total thyroxine (TT4) and thyroid stimulating hormone (TSH) were also analyzed and compared between two groups. Results The incidence of malnutrition in CHF patients was 41.7%. The level of TT3 was lower in the malnutrition group than that in the normal group (P〈0.01). There was no significant difference in TT4 and TSH levels between two groups. Conclusion This study reveal a relatively high incidence of malnutrition in severe CHF patients. There might be a relationship between changes in levels of thyroid hormone and malnutrition.
