摘　　要：目的评价微创经皮肾镜碎石术对肾内型肾盂结石的治疗效果。方法回顾性分析肇庆市第二人民医院2013年1月1日至2013年12月31日由同一术者完成的微创经皮肾镜碎石术187例,其中肾内型肾盂肾结石58例,非肾内型肾盂肾结石129例,比较二者之间的一期结石清除率、二期结石清除率、一期手术时间、手术中取石通道个数、二期体外冲击波碎石例数、输血率、胸膜损伤发生率。结果肾内型肾盂结石病例一期结石清除率低于非肾内型肾盂结石病例,而一期手术时间、双通道取石率均高于后者(P<0.05)。二期结石清除率、术后体外冲击波碎石例数、输血率、胸膜损伤发生率均无统计学差异(P>0.05)。结论微创经皮肾镜碎石术对肾内型肾盂合并结石的治疗安全有效。Objective To evaluate therapeutic outcomes of minimally invasive percutaneous nephrolithotomy for intrarenal pelvis calculi. Methods Date of 187 cases underwent minimally invasive percutaneous nephrolithotomy in our institution from Jan 1st to Dec 31st, 2013 were collectded. All operation were performed by one surgeon. Among these cases, 58 cases were intrarenal pelvis calculi, while the other 129 cases were non-intrarenal pelvis calculi. Restrospectively, the stone clearance rate, operation time, requirement for double-tract, postoperative extracorporeal shock-wave lithotripsy along with complicative rate of these two groups were compared. Results In the intraranal pelvis group, the stone clearance rate of one-stage operation was significantly lower comparied with non-intrarenal pelvis calculi group, although latter had shorter one-stage operation time and less requirement for double-tract （P〈0.05）. Nevetheless, there is no significant difference of the stone clearance rate of second-stage, requirement for postoperative ESWL, transfusion and injury of pleura between the two groups （P〉0.05）. Conclusion Minimally invasive percutaneous nephrolithotomy is a safe and effective operation to intrarenal pelvis calculi, but it has larger requirement for double-tract and stage.
