膀胱颈移动度在女性压力性尿失禁诊断中的应用价值

摘　　要：目的：探讨膀胱颈移动度在女性压力性尿失禁（SUI）诊断中的价值,为SUI的治疗提供理论依据。方法：选取2013年3月~2014年6月在该院妇科门诊就诊的女性254名,分为SUI组（146例）和对照组（108例）,应用经会阴超声测量静息状态和Valsalva状态下的膀胱颈至耻骨联合下缘的距离（BSD）并计算膀胱颈移动度,比较两组间膀胱颈移动度的差异,对膀胱颈移动度作诊断试验真实性评价。结果：SUI组的膀胱颈移动度大于对照组,差异有统计学意义（P〈0.05）。以膀胱颈移动度为SUI诊断标准,绘制ROC曲线,曲线下面积为0.894。SPSS分析ROC曲线坐标显示,当膀胱颈移动度的截断值为31.50 mm时,其对SUI的诊断价值最高,此时诊断的灵敏度为90.40%、特异度为78.70%、约登指数为0.69、诊断指数为169.10%、阳性似然比为4.24、阴性似然比为0.12。结论：膀胱颈移动度在女性SUI的诊断中具有较高的价值。Objective： To explore the value of mobility of bladder neck in diagnosis of female stress urinary incontinence（ SUI）,provide a theoretical basis for SUI treatment. Methods： A total of 254 women were selected from gynecological outpatient of the hospital from March 2013 to June 2014,then the women were divided into SUI group（ 146 women） and control group（ 108 women）,transperineal ultrasonography was performed to measure the distances between bladder neck and lower margin of symphysis pubis（ BSD） under quiescent condition and Valsalva condition,the differences of mobility of bladder neck were compared between the two groups,diagnostic test and authentic assessment of mobility of bladder neck were conducted. Results： Mobility of bladder neck in SUI group was statistically significantly larger than that in control group（ P〈0. 05）. Taking mobility of bladder neck as a diagnostic criterion for SUI,ROC curve was drawn,the area under the curve was 0. 894. When the cutoff value of mobility of bladder neck was 31. 50 mm,the diagnostic value was the highest,the sensitivity,specificity,Youden index,diagnostic index,the positive likelihood ratio and the negative likelihood ratio were 90. 40%,78. 70%,0. 69,169. 10%,4. 24 and 0. 12,respectively. Conclusion： Mobility of bladder neck has relatively high value in diagnosis of female SUI.
