摘  要:目的:探索胎儿心电图(FECG)与无应激实验(NST)联合应用在预测胎膜早破患者胎儿宫内窘迫中的作用。方法:选取嘉兴市妇幼保健院2013年1月~2013年12月因胎儿宫内窘迫而行剖宫产手术终止妊娠的73例患者为研究对象,其中34例胎膜早破患者为研究组,39例孕期产检无胎膜早破患者为对照组。回顾性分析相应的临床资料,比较两组患者的临床转归及新生儿结局。结果:所有入选病例NST均为无反应型,研究组检出FECG异常6例,其中术后证实胎儿宫内窘迫3例;对照组未检出FECG异常。与NST单一监测结果相比,NST与FECG联合应用较大地提高了判断胎儿宫内窘迫的特异度。结论:FECG与NST联合应用能提高预测胎膜早破患者胎儿宫内窘迫的准确性。Objective: To explore the role of fetal electrocardiography( FECG) combined with non stress test( NST) in predicting fetal intrauterine distress among pregnant women with premature rupture of fetal membrane( PROM). Methods: A total of 73 patients who terminated pregnancy because of fetal intrauterine distress via cesarean section in the hospital from January 2013 to December 2013 were selected as study objects,then the patients were divided into study group( including 34 patients with PROM) and control group( including 39 patients without PROM). The clinical data was analyzed retrospectively,the clinical outcomes and neonatal outcomes in the two groups were compared. Results: NST of all the patients was non- reactive. Six patients in study group were found with abnormal FECG,and three patients were diagnosed as fetal intrauterine distress after surgery; no one was found with abnormal FECG in control group. Compared with NST monitoring,NST combined with FECG significantly improved the specificity of judging fetal intrauterine distress. Conclusion: FECG combined with NST can improve the accuracy of predicting fetal intrauterine distress among pregnant women with PROM.
