我院临床药师参与307例疑难病例会诊分析  

Analysis on 307 Intractable Cases Consultations with Clinical Pharmacists' Participating in Our Hospital

摘　　要：目的:为提高临床药师专业素质,建立特色的药学服务提供参考。方法:对2012年6月-2013年12月临床药师参与我院临床会诊的疑难危重病例情况和质量进行统计分析。结果:临床药师参与全院会诊81例(26.38%),科间会诊226例(73.62%)。会诊主要涉及抗感染方案制订291例(71.34%),营养支持治疗23例(7.49%),围术期用药20例(6.51%)等;会诊科室覆盖骨科78例(25.41%)、神经内科30例(9.77%)、重症医学科29例(9.45%)等29个临床科室。会诊建议全部采纳有194例(63.19%),部分采纳有87例(28.34%),其中以抗感染治疗、营养支持治疗、围术期用药相关的会诊建议采纳率较高,而抗肿瘤药物应用相关会诊建议采纳率较低。结论:临床药师参与临床疑难病例会诊分析,可以临床需求为导向地提升临床药师专业素质,建立独具特色的药学服务。OBJECTIVE：To provide reference for the improvement of clinical pharmacist professional quality and establishment of characteristic pharmaceutical care. METHODS：The data of clinical pharmacists’ participating in clinical consultations difficult seriously ill patients and the quality of consultations from Jun. 2012 to Dec. 2013 were analyzed in our hospital. RESULTS：There were 81 cases（26.38%）were hospital consultations,226 cases（73.62%）were consultations between departments;219 cases（71.34%）were consultations mainly on anti-infection treatment,23 cases（7.49%）were nutrition support,20 cases（6.51%）were perioperative medication. In total of 29 clinical departments were involved in consultations,including Orthopedics 78 cases（25.41%）,Neurology 30 cases（9.77%）,ICU 29 cases（9.45%）and others. Clinical pharmacists’ consultation opinions were totally adopted in 194 cases（63.19%）and partly adopted in 87 cases（28.34%）. The adoption rate of opinions on anti-infection therapy,nutrition support therapy,perioperative medication was high. However,the adoption rate of opinions on application of antineoplastic medicament was low. CONCLUSIONS：Clinical pharmacists’ participating in clinical consultations can promote clinical pharmacists’ professional quality by the guide of clinical needs and create unique pharmaceutical care.
