摘　　要：目的:了解艾滋病高发区艾滋病病毒感染者及病人的费用负担及医疗保障现状。方法:选择艾滋病高发区云南省大理州为样本地区,选取样本地区艾滋病卫生服务提供者、管理者和艾滋病病毒感染者及病人共58人,从供、需、管理三方关键知情人视角就艾滋病医疗保障现状的认知进行调查。结果:绝大多数艾滋病病毒感染者及病人对医疗保障政策、救助政策及报销制度缺乏了解;艾滋病卫生服务提供方、管理方专家以及逾七成卫生服务利用方认为艾滋病患者疾病负担不重,但艾滋病治疗的报销范围过小。结论:现有医保报销比例合理但报销范围褊狭,政府对艾滋病患者常规检测投入不足,且艾滋病患者面临沉重心理负担。应加强对艾滋病相关医保及救助政策的宣传,扩大医疗保障报销范围,增加对艾滋病定点医疗机构常规检测的投入并加强正确认识艾滋病的宣传。Objective： To understand health care burden and medical insurance among People Living with HIV / AIDS in Dali Prefecture of Yunnan Province. Methods： An In- depth literature review among 58 administrators,providers and consumers of health service of HIV / AIDS was conducted at Dali,Yunnan Province. Results： The majority of respondents living with HIV / AIDS lacked enough understanding of medical insurance,and related reimbursement system. In the opinion of the administrators,providers and more than seventy percent of consumers,disease burden of PLWHA was not heavy but the reimbursement scope was too small. Conclusion： The current reimbursement rate is reasonable and the scope is narrow. Insufficient finance for routine testing is invested by the government,and PLWHA are facing heavy psychological burden. It＇s suggested that related departments should strengthen related medical insurance and assistance policy,expand the scope of medical insurance reimbursement,and improve the investment in routine testing.
