摘 要:目的:探讨调肝理脾方干预原发性胆汁性肝硬化肝组织损伤的作用机制。方法:建立C57BL/6小鼠原发性胆汁性肝硬化模型,观测肝组织病理、超微结构。结果:与正常组比较,模型组细胞角蛋白(CK19)阳性表达明显,胆管增生活跃,胆小管数量增多,管腔畸形,胶原纤维沉积明显;透射电镜显示胞质中含有大量脂滴,线粒体肿胀畸形,内质网囊性扩张、断裂,大量胶原纤维沉积,周边有嗜酸性粒细胞及浆细胞聚集。调肝理脾方组胆小管增生较模型组和阳性组明显减少,仅有少量增生的胶原纤维,CK19表达明显减弱;透射电镜显示胞质中偶见脂滴,胆管上皮细胞增生不明显,窦周间隙偶有少量增生的胶原纤维。结论:抑制胆小管的增生及胶原纤维的沉积可能是调肝理脾方有效干预原发性胆汁性肝硬化的重要机制之一。Objective: To explore the intervention mechanism of Tiaogan Lipi prescription on the hepatic tissue ultrastructure of mouse with primary biliary cirrhosis. Methods: C57 BL /6 mice were injected with cytidine polyinosinic acid to establish a primary biliary cirrhosis animal model. The ultrastructural changes of the liver tissues of the mice were observed in each group. Results: Compared to the normal group,the model group were obviously increased expression of CK19 accompanied with active bile duct proliferation,an increase in the numbers of bile canaliculi,lumen deformities and apparent collagen deposition. Transmission electron microscope was showed that the cytoplasm contained a large amount of lipid droplets. The mitochondria were swollen and deformed and the endoplasmic reticulum exhibited cystic expansion and even fracturing. Some of collagen fiber deposition were surrounded with collected eosinophils and plasmacytes. Fewer bile canaliculi,less collagen fiber hyperplasia and lower CK19 expression were observed in Tiaogan Lipi groups compared with the model group and ursodeoxycholic acid groups. Transmission electron microscope was showed that lipid drops occasionally appeared in the cytoplasm and the bile duct epithelial cell proliferation was not obvious. Few fibroblasts gathered around the perisinusoidal space. Conclusion: It is one of the important mechanisms of Tiaogan Lipi prescription to depress bile canaliculi hyperplasia and collagen fiber deposition in primary biliary cirrhosis.
