显微镜下射频消融低温等离子扁桃体切除术的应用研究

摘　　要：目的探讨显微镜下射频消融低温等离子切除扁桃体手术的临床应用效果。方法将2011年1月~2013年12月需行扁桃体切除术的180例患者随机分组,分别行显微镜下射频消融低温等离子扁桃体切除术（等离子组,n=90）和单极电刀电凝扁桃体切除术（电刀组,n=90）,比较两组的手术时间、术中出血量、术后疼痛程度、白膜脱落时间及术后再出血率。结果两组的白膜脱落时间比较差异无统计学意义（P〉0.05）;等离子组术中出血量、手术时间以及术后再出血率明显低于电刀组,差异有统计学意义（P〈0.05或0.01）,等离子组术后1 d、3 d、5 d的疼痛程度评分少于电刀组,差异有统计学意义（P〈0.01）。结论显微镜下行射频消融低温等离子扁桃体切除术具有手术时间短,术中及术后出血少,术后局部疼痛轻的优势,且操作简单、安全、有效,值得推广。Objective To explore clinical effects of low-temperature plasma radiofrequency tonsillectomy under microscope. Methods 180 cases who needed to undergo tonsillectomy from January,2011 to December,2013 were randomly divided into plasma group（ underwent low-temperature plasma radiofrequency tonsillectomy under microscope） and electrotome group（ underwent monopolar electrotome electrocoagulation tonsillectomy） with 90 cases in each group. Then,operation time,peri-operative bleeding,postoperative pain,time of albuginea drop and rate of postoperative recurrent bleeding were compared between groups. Results There was no statistically significant difference in time of albuginea drop between the two groups（ P〈 0. 05）; peri-operative bleeding,operation time and rate of postoperative recurrent bleeding of the plasma group were significantly lower than those of the electrotome group,so the difference were statistically significant（ P〈 0. 05 or 0. 01）;Scores of postoperative pain 1 d,3 d and 5 d after operation in the plasma group were lower than those of the electrotome group,so the difference were statistically significant（ P〈 0. 01）. Conclusion Low-temperature plasma radiofrequency tonsillectomy under microscope has the advantages of short operation time,less intraoperative and postoperative bleeding and slight local pain. It is a safe,effective and simple operation method,which is valuable to extend in clinic.
