摘　　要：目的研究社区人群中,外周血中性粒细胞计数与肱-踝脉搏波传导速度的相关性。方法于2010年3月至8月采取整群抽样的方法,在上海市嘉定区纳入40岁以上居民239名,分析其外周血中性粒细胞水平与肱-踝脉搏波传导速度的相关性。将人群中肱-踝脉搏波传导速度〉1400 cm/s定义为动脉硬化。结果血管硬化组中性粒细胞计数水平显著高于非硬化组（3.67×10^9/L对3.44×10^9/L,P=0.0001）。Pearson相关分析表明,中性粒细胞计数、年龄、收缩压、舒张压、空腹血糖、口服葡萄糖耐量试验（OGTT）2 h血糖、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、体重指数、高密度脂蛋白胆固醇均与肱-踝脉搏波传导速度相关（P均〈0.05）。校正性别、年龄、体重指数、收缩压、OGTT 2h血糖、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇后,中性粒细胞计数与肱-踝脉搏波传导速度呈显著正相关（r=17.71,P〈0.0001）。将肱-踝脉搏波传导速度〉1400 cm/s定义为血管硬化,校正了除血压外的因素后,中性粒细胞计数的最高四分位组（〉4.238×10^9/L）相对于最低四分位组,血管硬化的患病风险增加39%（OR=1.39,95%CI 1.02-1.88,P〈0.05）。然而,在校正动脉血压后,其相关性消失。结论在社区40岁以上人群中,中性粒细胞计数升高与血管硬化呈显著正相关,中性粒细胞是血管硬化的独立危险因素,这种相关性不独立于动脉血压。Objective To investigate the relationship between peripheral neutrophil count and pulse wave velocity（PWV） in a middle-aged and elderly Chinese population. Methods This cross-sectional study included 2395 middle-aged and elderly subjects from Shanghai,China. PWV were determined by ba PWV. Results The peripheral neutrophil count levels in all subjects were positively correlated with PWV（r = 17.71,P〈0.0001） after being adjusted for age,gender,body mass index（BMI）,systolic blood pressure（SBP）,high-density lipoprotein（HDL）,low-density lipoprotein（LDL）,triglyceride（TG）,total cholesterol（TC） level and oral glucose tolerance test（OGTT）-2 h plasma glucose（2h-PG）. A logistic regression analysis,adjusted for age,sex,BMI,2h-PG,LDL,HDL,TG,TC,revealed that compared with the lowest quartile of the neutrophil count,the highest quartile were significantly associated with the risk of arteriosclerosis（adjusted OR = 1.39,95%CI 1.02-1.88,P 0.05）. But after adjustment for SBP,the association was not significant. Conclusion In a middle-aged and elderly Chinese population,the elevated neutrophil count level is associated with ateriosclerosis after adjustments of conventional cardiovascular risk factors,but the result is not independent of SBP,implying that neutrophil count might be an independent factor of arteriosclerosis.
