摘 要:目的探讨卵泡液和胚胎培养液中白血病抑制因子(LIF)的表达水平与体外受精-胚胎移植(IVF-ET)结局的关系。方法收集取卵日卵泡液、胚胎移植及冷冻后胚胎培养液,检测卵泡液中LIF、雌二醇(E_2)、孕酮(P)和胚胎培养液中LIF的水平,分析卵泡液和胚胎培养液LIF与卵子质量、胚胎质量及临床结局的关系。结果妊娠组胚胎培养液中LIF浓度显著高于未妊娠组(t=5.785,P<0.05),而妊娠组卵泡液P浓度显著低于未妊娠组(t=4.475,P<0.05)。两组卵泡液LIF、E_2浓度差异无统计学意义(t值分别为1.305、0.680,均P>0.05)。卵泡液LIF浓度与获卵数、成熟卵子率、卵裂率、优质胚胎率、E_2均呈显著正相关(r值分别为0.285、0.394、0.302、0.387、0.476,均P<0.05),卵泡液LIF与受精率、卵泡液P无显著相关性(r值分别为0,050、0.243,均P>0.05)。卵泡液LIF与移植胚胎胚胎培养液中LIF无显著相关性(r=0.107,P>0.05)。优质胚胎培养液中LIF水平显著高于劣质胚胎培养液中LIF水平(t=8.334,P<0.05)。结论卵泡液中LIF与E_2、卵母细胞质量有关,可以根据卵泡液中LIF评估卵母细胞的质量。胚胎培养液中LIF可以作为预测胚胎质量和妊娠结局的依据。Objective To explore the relationship between the level of leukaemia inhibitory factor (LIF) in follicular fluid and embryo culture media and the outcomes of in vitro fertilization and embryo transfer ( IVF-ET ). Methods Follicular fluid on the day of ooeyte retrieval and embryo culture media after transfering and freezing were collected. The level of LIF, estradiol ( E2 ) , progesterone (P) in follicular fluid and level of LIF in embryo culture media were measured to analyze the relationship among LIF with the quality of oocyte, embryo and the outcomes of IVF-ET. Results The level of LIF in embryo culture media was significantly higher and the level of P in follicular fluid was lower in the pregnant group than in non-pregnant group ( t value was 5. 785 and 4.475, respectively, both P 〈 0. 05 ). But there was no significant difference in the levels of LIF and E2 in follicular fluid between two groups (t value was 1. 305 and 0. 680, respectively, both P 〉 0.05 ). The level of LIF in follicular fluid was positively correlated with the number of retrieval oocyte, the rate of mature oocyte, cleavage rate, high quality embryo rate and E2 (r value was 0. 285, 0. 394, 0. 302, 0. 387 and 0. 476, respectively, all P 〈 0.05), but it was not correlated with fertility rate and P ( r value was 0. 050 and 0. 243, respectively, both P 〉 0.05). There was no obvious correlation between LIF in follicular fluid and LIF in embryo culture media after transferring (r = 0. 107, P 〉 0.05 ). The level of LIF in embryo culture media of high quality was significantly higher than poor quality (t = 8. 334 ,P 〈 0.05 ). Conclusion The level of LIF in follicular fluid is correlated with E2 and the quality of oocyte, and the quality of oocyte can be assessed according to the level of LIF. The LIF in embryo culture media can be used to predict the quality of embryo and outcomes of IVF-ET.
