摘  要:目的探讨颅脑外伤恢复期下丘脑-垂体-靶腺激素水平异常的发生率。方法用放射免疫法测定患者伤后3、6和12个月周围静脉血中游离T3(FT3)、游离T4(FT4)、促甲状腺激素(TSH)、生长激素(GH)、促肾上腺皮质激素(ACTH)、皮质醇(Cor)、泌乳素(PRL)、睾酮(T)、雌二醇(E2)、卵泡刺激素(FSH)、黄体生成素(LH)、孕酮(P)的浓度,统计分析颅脑外伤恢复期下丘脑-垂体-靶腺激素异常的发生率及其与恢复时间及病情严重程度的关系。结果激素异常发生率和损伤程度相关,损伤越重,激素异常发生率越高,差异有统计学意义(P<0.05)。激素异常发生率随着时间推移逐渐降低,伤后3个月为19.9%,6个月为17.6%,12个月为14.8%,但差异无统计学意义(P>0.05)。结论颅脑外伤恢复期神经内分泌激素水平异常的发生率,随着恢复时间的延长有下降趋势,可作为恢复期一项重要的检测指标,用于指导临床治疗。【Objective】 To study the incidence of abnormal hypothalamic-pituitary-target gland hormones in the recovery phase of the patients with traumatic brain injury. 【Methods】 The concentrations of FT3, FT4,TSH, GH, ACTH, Cor, PRL, T, E2, FSH, LH, P in the peripheral blood were measured by radioimmunoassay3, 6 and 12 months after craniocerebral trauma. The incidence of the abnormal hypothalamic-pituitary-target gland hormones and its relation to the recovery time and disease severity were statistically analyzed. 【Results】 The incidence of neuroendocrine hormone abnormalities was positively related to the injury severity(P〈0.05). The hormone abnormal rate gradually decreased with time, was 19.9% 3 months after injury, 17.6% 6months after injury and 14.8% 12 months after injury, but the differences were not statistically significant(P〉0.05). 【Conclusion】 In the recovery phase, some patients with traumatic brain injury have neuroendocrine hormone abnormalities, the hormone levels can be used as important indexes to guide clinical therapy.
