不同类型勃起功能障碍患者血清睾酮水平变化的相关研究

摘　　要：目的：探讨不同类型勃起功能障碍（ED）患者血清睾酮水平的变化，为ED患者的诊断和治疗提供重要的临床依据。方法应用夜间阴茎勃起监测仪检查，根据其夜间阴茎勃起时阴茎硬度平均压力的变化情况，将患者区分为A组（心理性ED）和B组（器质性ED）两组，另设C组为正常对照组。应用酶联免疫法进行血清睾酮水平测定。结果夜间阴茎勃起试验（NPT）监测：有效勃起次数、平均张力及平均勃起时间A组分别为（4.23±1.49）次、（129.82±22.35）g、（498.52±206.13）s，B组的分别为（1.45±0.22）次、（94.51±19.42）g、（158.34±30.26）s；C组分别为（4.82±1.26）次、（130.14±20.33）g、（511.12±210.16）s。血清睾酮水平测定： A组（26.63±9.24） nmol/L，B组（20.31±8.46）nmol/L，C组（27.38±8.35） nmol/L。 A、C两组的血清睾酮水平明显高于B组（P＜0.05），A、C两组的夜间有效勃起次数、平均勃起时间、阴茎平均张力优于B组（P＜0.05）。血清睾酮水平与NPT的勃起次数、平均勃起时间、平均张力相关系数分别为0.329、0.368、0.285，均有明显相关性。结论器质性ED患者的血清睾酮水平明显低于心理性ED患者及正常男性，NPT监测指标与血清睾酮水平呈正相关，因此血清睾酮对ED临床分类及睾酮补充治疗有一定的指导意义。Objective To study the serum levels of testosterone in patient with erectile dysfunction, and provide clinical reference for the ED diagnosis and therapy. Methods All subjects were divided into three groups based on the penis mean tension during the NPT measuring such as group A（ the psychological ED）, group B（the organic ED） and group C （normal contral group）. The level of serum testosterone was detected by Enzyme-linked immunosorbent assay. Results The NPT measuring parameters of group A and C were all better than those of group B. The level of serum testosteronein group A, B and C was （26.63±9.24）nmol/L, （20.31±8.46）nmol/L, （27.38±8.35）nmol/L respectively. There was significance difference amonggroup A, C and B in level of serum testosterone（P〈0.05）. Serum testosterone level was positively correlated with effective erection numbers （r=0.329）, average erectile time（r=0.368）, penis mean tension （r=0.285）. Conclusion The level of serum testosterone in organic ED patients was obviously lower than that in the psychological ED patients and normal group. There was a positive correlation between serum testosterone level and NPT measuring parameter. Analysis of serum testosterone level will provide a guiding significance for ED diagnosis and therapy in clinic.
