摘　　要：分析垂体瘤各种术式下的并发症。回顾性分析自2004年1月至2014年1月采用显微镜下开颅手术、经口鼻蝶、单鼻腔经蝶及内镜经蝶切除265例垂体瘤患者的并发症。结果开颅手术36例、经口鼻蝶113例、单鼻腔经蝶109例、内镜经蝶19例,包括5例为二次手术患者。术后78例患者出现并发症,其中尿崩62例、脑脊液鼻漏35例、视力障碍加重4例、垂体低功58例、离子紊乱45例,术中大出血3例,死亡2例。垂体瘤各术式均安全可靠,但仍可根据各术式特点采取相应方法减少并发症。To evaluate the complications related to different approaches for pituitary adenoma surgery . A retrospective analysis was conducted on 265 patients with pituitary adenomas who underwent microscopic transcranial ,transoral‐nosal‐sphenoidal ,single‐nostril transsphenoidal or endoscopic transsphenoidal approach between January 2003 and January 2013 . The potential causes of complications were studied . 36 patients underwent microscopic transcranial surgery , 113 microscopic transoral‐nosal‐sphenoidal surgery , 105 microscopic single‐nostril transsphenoidal surgery , 19 endoscopic transsphenoidal surgery ,among them five patents with 2 staged surgery .The complications were found in 78 patients . The complications included diabetes insipidus in 62 patients ,CSF leakage in 35 patients ,visual impairment in 4 patients , hypothyroidism in 58 patients ,electrolyte disturbances in 45 patients ,bleeding in 3 patients and 2 patients died .All the approaches are safe and effective .The complications can be further decreased through different methods corresponding to each approach respectively .
