群组管理对脑卒中吞咽障碍患者康复训练效果的影响

摘　　要：目的探讨群组管理对脑卒中吞咽障碍患者康复训练效果的影响。方法 150例脑卒中吞咽障碍患者随机分为观察组和对照组,每组各75例。在常规药物治疗的基础上,对照组采取常规的一对一康复训练模式,观察组采取群组管理模式进行康复训练,时间均为5周。比较两组患者干预前后的总体疗效、美国国立卫生研究院脑卒中量表(NIHSS)和标准吞咽功能评价量表(SSA)评分。结果观察组的总有效率明显高于对照组(82.7%vs 68.0%,P<0.05)。干预后两组患者的NHISS评分和SSA评分均较干预前明显下降(P<0.05),并且观察组干预后的NHISS评分和SSA评分明显低于对照组[(9.2±2.1)分vs(10.3±2.2)分,(23.1±4.5)分vs(25.1±4.8)分,P均<0.05]。结论与常规的一对一模式相比,群组管理模式对脑卒中吞咽障碍患者的康复训练效果更突出,神经功能和吞咽功能的恢复更佳,值得临床推广。【Objective】To investigate the effect of group management on the rehabilitation training of stroke patients with dysphagia. 【Methods】A total of 150 stroke patients with dysphagia were randomly divided into the observation group and the control group. Based on conventional medication, the 75 cases in the control group were given conventional one-to-one rehabilitation training mode while the 75 cases in the observation group were given group management mode. All were trained for 5 months. The curative effect, the scores of the National Institute of Health Stroke Scale（NIHSS） and the Standardized Swallowing Assessment Scale（SSA） of the two groups were compared before and after the training.【Results】The total effective rate of the observation group was significantly higher than that of the control group（82.7% vs. 68.0%, P〈0.05）. The scores of NIHSS and SSA of all the cases decreased significantly after the intervention（P〈0.05）, and the scores of NIHSS and SSA of the observation group were significantly lower than those of the control group after intervention [（9.2 ±2.1） vs.（10.3 ±2.2）,（23.1 ±4.5） vs.（25.1 ±4.8）, all P〈0.05].【Conclusions】Compared with conventional one-to-one mode, the effect of group management mode on rehabilitation training of stroke patients with dysphagia is more prominent, the recovery of nerve and swallowing function is better. It should be promoted.
