应用组织多普勒成像评价扩张型心肌病右室功能

摘　　要：目的探讨应用组织多普勒成像技术(TDI)评价扩张型心肌病(DCM)患者右室功能的临床价值。方法 DCM患者84例,根据心室的大小,分为左心型、右心型及全心型三组。健康对照组50例,取标准心尖四腔心切面,应用TDI定量分析三尖瓣前叶及隔叶瓣环处的组织运动速度。结果与正常组比较,三组DCM患者三尖瓣环处组织运动VS、VE、VE/VA均明显减低,差异有统计学意义(P<0.05),并以全心型组最为显著。三组DCM患者间两两比较,全心型后间隔瓣环处VS高于右心型,左心型侧壁瓣环处VE/VA与全心型比较差异无统计学意义(P>0.05),余VS、VE、VE/VA表现为全心型<右心型<左心型,差异有统计学意义(P<0.05)。结论 TDI速度指标VS能快速评价DCM患者右室收缩功能,而VE、VE/VA能快速评价DCM患者右室舒张功能,具有重要的临床价值。【Objective】To evaluate the clinical value of tissue Doppler imaging（TDI） in detecting the right ventricular function of dilated cardiomyopathy（DCM）.【Methods】Totally 84 patients with DCM and 50 normal controls were studied by TDI. The patients with DCM were divided into three groups which were the left ventricle group, the right ventricle group and the all ventricle group by the size of the ventricle. And the standard apical four cavity section was obtained, the indices of tricuspid valve annulus in the lateral wall of the right ventricle and the post-interventricular septum were detected by TDI, then compared. The indices included systolic peak velocity（VS）, early-diastolic peak velocity（VE）, late-diastolic peak velocity（VA）, ratio of VE and VA（VE/VA）.【Results】Compared with the normal controls, VS, VE and VE/VA were decreased significantly in tricuspid valve annulus with DCM（P〈0.05）,and were the lowest in the all ventricle group. Compared with each other in the DCM groups, VS of tricuspid valve annulus of the post-interventricular septum was higher in the all ventricle group than in the right ventricle group,there was no significant difference in VE/VA of tricuspid valve annulus between the left ventricle group and the all ventricle group（P〉0.05）. The others of VS, VE, VE/VA were lower in the all ventricle group than in the right ventricle group, and which were in turn lower than in the left ventricle group（P〈0.05）.【Conclusions】VS can quickly evaluate the right ventricular systolic function in the patients with DCM, while VE and VE/VA can quickly evaluate the right ventricular diastolic function. Thus TDI has an important clinical value.
