颈部物理降温在减少咽喉部激光手术患者恢复期躁动中的作用

摘  要:目的观察颈部物理降温对咽喉部激光手术患者术后疼痛和恢复期躁动发生的影响,并探讨其在减少恢复期躁动中的作用。方法 120例择期接受咽喉部激光手术患者,随机分为3组(n=40):对照组(A组)、颈部常温组(B组)、颈部降温组(C组),在恢复期对照组无其它处理,常温组患者安放冷敷带,但不冷敷,降温组患者进行颈部冷敷。记录患者心率、平均动脉压和脉氧饱和度,观察患者自主呼吸恢复、意识恢复及恢复室停留时间,并对患者进行躁动和疼痛评分。结果与C组相比,A组与B组患者完全苏醒时心率增快(P<0.05)、血压增高(P<0.05);3组患者呼吸恢复时间、意识清醒的时间差异无统计学意义,但C组患者恢复室停留时间短于A组(P<0.05)和B组(P<0.05);C组患者恢复期躁动及疼痛发生例数和发生率均低于A组(P<0.05)和B组(P<0.05)。结论颈部物理降温可明显减轻咽喉部激光手术患者的术后疼痛,同时减少恢复期躁动的发生。【Objective】To observe the influence of neck physical cooling on pain and agitation of throat laser operation patients, and to analyze the effects of neck physical cooling on reducing agitation. 【Methods】One hundred and twenty throat laser operation patients were divided into three groups(n =40): control group(group A), neck common temperature group(group B) and neck physical cooling group(group C). The patients in group A were with no special treatment, the patients in group B were given old compress and without cooling, the patients in group C were executed neck physical cooling. The HR, MAP and Sp O2 were recorded. The spontaneous breathing recovery time, consciousness recovery time and recovery room detain time were observed. At last the pain and agitation scores were completed.【Results】The recovery room stay time in group A and group B were longer than group C(P〈0.05).The pain and agitation incidence in group A and group B were higher than group C(P〈0.05). But there was no significantly difference between group A and group B(P〉0.05). 【Conclusion】Neck physical cooling can relieve the pain of throat laser operation patients and reduce agitation during recover period.
