消化性溃疡出血患者外周血骨代谢指标的水平与幽门螺杆菌感染的关系

摘　　要：目的探讨消化性溃疡出血患者血清Ⅰ型胶原氨基端前肽(PINP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTX)的水平变化及与幽门螺杆菌(Hp)感染的关系。方法选择2011年1月-2013年12月80例消化性溃疡患者为观察组,分为溃疡出血组50例和溃疡组30例,选择36名健康者为对照组,检测受试人员血清中PINP、β-CTX含量,并对各指标与年龄、病程、临床症状、Hp感染等关系进行相关性分析。结果观察组患者血清PINP、β-CTX含量明显低于对照组,差异有统计学意义(P<0.01);溃疡出血组患者血清PINP、β-CTX水平明显低于溃疡组和对照组(P<0.05),溃疡组血清PINP、β-CTX含量低于对照组(P<0.05);Hp阳性患者血清PINP、β-CTX含量低于Hp阴性患者(P<0.05)。结论 PINP、β-CTX在消化性溃疡出血患者血清中呈低表达;PINP、β-CTX在消化性溃疡出血中的表达与患者Hp感染状况有关。OBJECTIVE To explore the relationship between the change of level of serum type Ⅰ collagen amino-terminal propeptide （PINP）or type Ⅰ collagen carboxy-terminal peptide β special sequence （β-CTX） and the Heli-cobacter pylori infection in the patients with peptic ulcer bleeding .METHODS A total of 80 patients with peptic ul-cer bleeding who were treated in the hospital from Jan 2011 to Dec 2013 were assigned as the observation group and were divided into the ulcer bleeding group with 50 cases and the ulcer group with 30 cases ,meanwhile ,36 healthy people were set as the control group .The contents of the serum PINP and β-CTX of the participants were determined ,and the relationship between the related indicators and the age ,illness course ,clinical symptoms ,and H .pylori infection was observed .RESULTS The levels of the serum PINP and β-CTX of the patients in the obser-vation group were significantly lower than those of the patients in the control group （P〈 0 .01） ;the levels of the serum PINP and β-CTX of the patients in the ulcer bleeding group were significantly lower than those of the pa-tients in the ulcer group and the patients in the control group （P〈 0 .05） ;the levels of the serum PINP and β-CTX of the patients in the ulcer group were significantly lower than those of the patients in the control group （P 〈0 .05） ;the levels of the serum PINP and β-CTX of the patients tested positive for H .pylori were significantly lower than those of the patients tested negative （P〈 0 .05） .CONCLUSION The PINP and β-CTX show the low ex-pression in the serum of the patients with peptic ulcer bleeding ,and the expression of the PINP or β-CTX in the serum of the patients with peptic ulcer bleeding is associated with the H .pylori infection .
