中心静脉导管静脉炎治疗的疗效研究

摘　　要：目的：研究超短波理疗联合京万红药膏外敷治疗经外周静脉置入中心静脉导管（PICC）化疗引起静脉炎的临床疗效。方法选取2013年6月－2014年6月82例行 PICC 化疗引起静脉炎的患者作为研究对象，按照随机分组法将其分为对照组和观察组，每组各41例，对照组给予33％硫酸镁溶液湿热敷病变部位，每日1次，连续使用3～5 d ；观察组患者给予超短波理疗联合京万红药膏外敷，每日1次，连续使用3～5 d ，并观察对比两组患者治疗后临床疗效、静脉炎临床症状改善及不良反应。结果观察组患者显效26例、好转15例，总有效率100．0％；对照组显效22例 、好转13例、无效6例，总有效率为85．4％，观察组临床疗效明显高于对照组（P＜ 0．05）；观察组在红肿消退时间、消退面积、疼痛缓解时间和血管弹性恢复等显著优于对照组，差异有统计学意义（P＜ 0．05）。结论超短波理疗联合京万红药膏治疗 PICC 化疗静脉炎临床效果明显，并具有简便、经济、明显缩短治疗疗程等特点，该方法值得在临床推广应用。OBJECTIVE To observe the clinical efficacy of ultrashort infrared physiotherapy combined with topical use of Jingwanhong ointment in treatment of phlebitis induced by peripherally inserted central venous catheter （PICC） chemotherapy .METHODS From Jun 2013 to Jun 2014 ,a total of 82 patients with phlebitis induced by PICC chemotherapy were recruited as the study objects and randomly divided into the control group and the obser -vation group ,with 41 cases in each .The control group was treated with 33% sulfuric acid magnesium solution for moist and heating of lesion site ,once per day ,continuous use for 3 to 5 days ;the observation group was given the ultrashort infrared physiotherapy combined with topical use of Jingwanhong ointments ,once per day ,continuous use for 3 to 5 days .The clinical effects ,improvement of clinical symptoms of phlebitis ,and adverse reactions were observed and compared between the two groups .RESULTS Totally 26 cases were effective and 15 cases were im-proved in the observation group ,with the total effective rate of 100 .0% ;totally 22 cases were effective ,13 cases were improved ,and 6 cases were ineffective in the control group ,with the total effective rate of 85 .4% ;the clini-cal efficacy of the observation group was significantly higher than that of the control group （P〈 0 .05） .The swell-ing subsided time ,subsided area ,pain relief time ,and elastic recovery of blood vessels of the observation group were significantly better than those of the control group （P〈 0 .05） .CONCLUSION The ultrashort infrared physio-therapy combined with topical use of Jingwanhong ointment can achieve significant clinical effect on treatment of phlebitis induced by PICC chemotherapy and is characterized by simple operation ,economical saving ,and short treatment course ,and it is worthy to be promoted in the hospital .
