摘　　要：目的:探讨团体治疗在卒中抑郁患者日常生活能力训练中的作用。方法:选取脑卒中后住院患者249名,汉密尔顿抑郁量表(Hamilton Depression Scale,HAMD)筛选出62名卒中后抑郁患者,62名患者被分为对照组(组1)和治疗组(组2),每组31人。两组均给予常规治疗,治疗组在此基础上定期开展开放式团体治疗。两组患者分别在治疗前、治疗后第12周采用HAMD、改良Barthel指数量表(Modified Barthel index,MBI)测评。结果:团体治疗12周后,两组HAMD评分均显著下降(t=14.228,4.385;P<0.001),治疗后治疗组改善明显优于对照组(t=-7.417,P<0.001),两组MBI评分均显著升高(t=-28.570,-20.135;P<0.01),且治疗组改善明显优于对照组(t=10.416,P<0.01)。结论:团体治疗能显著改善卒中抑郁患者的抑郁情绪并能提高其日常生活能力,值得临床推广使用。Objective：To investigate the therapeutic effect of group psychotherapy on ADL abilities in post-stroke depression（ PSD） patients.Methods：249 first-episode stroke patients from June 2012 to December 2013 were screened by Hamilton Depression Scale（HAMD）,62 patients with PSD were ran-domly divided into two groups,a control group（Group1：31 cases） treated with conventional treatment a-lone and a treatment group（ Group2：31 cases） which was treated by both conventional treatment and reg-ular open group psychotherapy.HAMD and MBI scales were used to evaluate the patients in each group before the start of the treatment and 12 weeks later.Results：The scores of HAMD decreased（t=14.228, 4.385;P〈0.001）in two groups,and the treatment group was lower than the control group（t=-7.417,P〈0.001 ）;These scores of MBI increased significantly after 12 weeks in both groups （ t =-28.570,-20.135;P〈0.01）,especially in the treatment group（t=10.416,P〈0.01）.Conclusion：Group psy-chotherapy can improve not only ADL abilities,but also the depressive mood in patients with PSD,and it＇s worthy to be widely spread in clinical condition.
