颈动脉支架术对远期血压的影响及相关因素分析

摘　　要：目的探讨颈动脉支架成形术对患者术后远期血压的影响,并分析其可能的影响因素。方法回顾分析我院73例颈内动脉支架成形术患者的临床资料,分析对比术后3-6个月与术前的血压变化特征。按照收缩压是否下降分为2组,分析性别、年龄、术中低血压史、高血压史、糖尿病史、支架位置、合并其他颈部血管狭窄及残余狭窄程度等因素,行非条件性Logistic逐步回归分析。结果所有患者术后3-6个月收缩压、舒张压、降压药使用种类与术前相比,差异有统计学意义（P〈0.05）;术后3-6个月心率与术前相比,差异无统计学意义（P=0.717）。多因素Logistic回归分析发现年龄、术中低血压、高血压及合并其他颈部血管狭窄与患者远期收缩压下降有关。结论颈内动脉支架成形术对患者远期心率无影响,可降低远期收缩压和舒张压。影响患者远期收缩压的下降的危险因素分别是年龄大、术中发生低血压、无高血压史及无合并其他颈部血管狭窄。Objective To investigate the correlation factors analysis and effects of carotid artery angioplasty and stenting（CAS）on the long-term blood pressure of patients.Methods The clinical data of 73 patients with CAS was collected to analyze the blood pressure change before operation and 3 to 6 months after operation.The gender,age,intraoperative hypotension and hypertension history,diabetes mellitus,stent position,cervical vascular stenosis and residual stenosis degree of patients were analyzed by non-conditional Logistic stepwise regression analysis.Results There was no significant difference in the heart rate（P=0.717）,but significant differences in the levels of systolic blood pressure,diastolic blood pressure and antihypertensive drugs number between preoperative and postoperative 3-6 months（P〈0.05）.Multivariate Logistic regression analysis showed that the change of long-term systolic blood pressure might be associated with age,intraoperative hypotension and hypertension history,and cervical vascular stenosis.Conclusion CAS has no effect on heart rate but can lower the long-term systolic and diastolic blood pressure.The risk factors affecting the decline of long-term blood pressure are older age,intraoperative hypotension,no hypertension and cervical vascular stenosis.
