摘 要:目的:用长春新碱(vincristine,VCR)建立家兔化疗性静脉炎(chemotherapeutic phlebitis,CP)模型。方法:42只日本大耳白兔随机分成3组:对照组、0.01%VCR组和0.02%VCR组,每组14只,分别从耳缘静脉注射0.9%NaCl注射液2ml/kg、0.01%VCR 0.2mg/kg和0.02%VCR 0.2mg/kg。根据给药速度不同(左耳12ml/h、右耳6ml/h),每组又分为A、B两个亚组。用数字皮温计测定给药前和给药后不同时间的兔耳缘皮温;肉眼观察外观变化,并根据严重程度记分。于给药后48、96h随机各取7只家兔处死。取耳缘组织切片,光镜下观察病理改变,记录每个标本各种病理变化的分值,并计算每个标本炎症反应积分。结果:与对照组比较,兔耳皮温在给药后12h开始下降,变化可持续到给药后48h,甚至84h(P<0.01,P<0.05)。注射局部肉眼观察及组织病理学观察可见:相同浓度的VCR,注射速度慢比注射速度快引起的CP更严重,而注射速度相同时,低浓度VCR引起的CP比高浓度严重。结论:兔耳缘静脉以6ml/h注射0.01%VCR 0.2mg/kg建立的CP最严重。Objective:To develop a rabbit model of chemotherapeutic phlebitis(CP)induced by vincristine(VCR).Methods:Forty-two Japanese rabbits were randomly divided into 3groups:the control group,the 0.01% VCR group and the0.02% VCR group,each consisting of 14 animals.The animals in each group were intravenously administered in the vein of the ear edge respectively with normal saline(0.9% NaCl)at a dosage of 2 ml/kg,0.01% VCR at a dosage of 0.2 ml/kg and0.02% VCR at a dosage of 0.2ml/kg.Depending on the rate of infusion(12 ml/h for the left ear and 6 ml/h for the right ear),the experimental groups were further divided into 2subgroups.Dermal temperature at the edge of the ear was detected with digital dermal thermometer at various time points both before and after medication.Outward appearance was observed with naked eye and scores were recorded in accordance with the severity of symptoms.Seven rabbits randomly chosen from the3 animals groups were sacrificed at 48 hand 96hafter medication.Samples were collected from the ear tissue for pathological changes under electron microscope,fractional values of pathological changes in each sample were recorded,and inflammatory response scores for each sample were calculated accordingly.Results:As compared with that of the control group,dermal temperature in the ear began to descend at 12 hafter medication,and this descending trend could continue till 48 hor even 84 hafter medication(P〈0.01,P〈0.05).Visual and histopathological observations revealed the following facts:at the same dosage,CP at injected sites induced by slow injection rate seemed to be more serious,when compared with that done by rapid injection rate,while at the same injection rate,CP induced by high-concentration VCR was more serious,as compared with that induced by low-concentration VCR.Conclusion:CP at the edge of the rabbit ear induced by an injection rate of 6ml/h and with dosages of 0.01% VCR and 0.2mg/kg was the most serious.
