中药结合脉冲射频治疗三叉神经痛临床研究

摘　　要：目的:观察中药及脉冲射频对三叉神经痛的临床疗效。方法:将42例三叉神经痛患者分为两组对照组21例采用中药治疗三叉神经痛,治疗组21例采用中药结合脉冲射频治疗三叉神经痛。观察治疗前及治疗后1周、1个月、6个月及1 a随访时的疼痛视觉模拟(visual analogue scale/Score,VAS)评分、生存质量(quality of life,QOL)评分、简式疼痛问卷表(short-form of M-c Gill pain questionnaire,SF-MPQ)评分、病变皮肤麻木评分、疼痛缓解有效率比较。结果:两组患者的VAS评分、QOL评分及SF-MPQ评分均较治疗前明显降低(P<0.05)。两组相比,VAS评分、SF-MPQ评分及QOL评分治疗组在治疗后1周、1个月、6个月和1 a随访时较对照组明显降低(P<0.05);治疗组在给予治疗后1周、1个月、6个月、1 a时的疼痛缓解率较对照组有显著差异(P<0.05)。麻木评分比较差异无统计学意义。结论:脉冲射频可能是通过激活疼痛感受抑制系统及调节三叉神经功能而起到止痛效果,中药长期服用可能是缓解患者动脉痉挛和神经的敏感性而起到治疗作用,所以二者结合治疗效果更明显。Objective： To observe the effect of Traditional Chinese medicine combined with pulse in treating trigeminal. Methods： 42 patients with trigeminal neuralgia were divided into two groups. 21 cases of control group were given traditional Chinese medicine. 21 cases of treatment group were given traditional Chinese medicine combined with pulse. Observe the visual analog pain score method（ visual analogue scale / Score,VAS）,quality of life Score（ QOL）,short-form of M-c Gill pain questionnaire,skin numbness score and pain relief effectiveness in 1 week,1 month,6 months and 1 year follow-up. Results： The VAS score,QOL score and SF-MPQ scores of two groups were significantly lower than before treatment（ P〈0. 05）. Compared between groups,the VAS score,SF-MPQ score and QOL score of group B after 1 week,1 month,6 months and 1 year were significantly lower than that of group A（ P〈0. 05）. Curative effect evaluation of pain relief of group B after treatment 1 week,1 month,6 months and 1 year rate of group A had statistical significance compared with group A（ P〈0. 05）. Numbness score had no statistical significance. Conclusion： Pulse radio frequency may be by activating pain feelings suppression system and regulating function of trigeminal nerve to achieve analgesic effect. Long-term use of Chinese traditional medicine may relieve patients with arterial spasm and the sensitivity of nerve to achieve treatment effect. So the combination of the two treatments has more obvious effect.
