多层螺旋CT后处理技术在肋骨隐匿性骨折诊断中的应用价值  被引量:38

摘  要:目的讨论多层螺旋CT(MSCT)后处理技术在肋骨隐匿性骨折诊断中的应用价值。方法回顾性分析2010年1月至2013年5月绵竹市人民医院放射科收治的85例高度可疑肋骨骨折患者的临床资料,经过MSCT扫描后,应用syngo Multi Modality工作站提供的最大密度投影(MIP)、多层面重建(MPR)、表面遮盖显示(SSD)、容积再现技术(VR)4种图像处理方法进行图像重建,分析比较不同的图像算法针对隐匿性骨折的诊断准确率。结果 VR相共检出87根隐匿性骨折,总检出率为91.6%;MIP相共检出94根,总检出率为98.9%;MPR相共检出87根,检出率为91.6%;SSD相共检出80根,检出率为84.2%,各组比较差异有统计学意义(P<0.05)。MIP总检出率显著高于VR、MPR、SSD(P<0.05)。结论对于X线平片没有诊断为骨折的患者,可以行MSCT检查,结合强大的图像后处理功能,能够显著提高隐匿性骨折的诊断准确性,为合理选择治疗方案提供参考依据。Objective To discuss the diagnostic value of multi-slice spiral CT( MSCT) post-processing technology in occult rib fracture. Methods Clinical data of 85 patients with highly suspicious rib fractures admitted to Radiology Department,Mianzhu People's Hospital from Jan. 2010 to May 2013 were retrospectively analyzed. After MSCT scanning,images of patients were reconstructed using maximum intensity projection( MIP),multi-planar reconstruction( MPR),shaded surface display( SSD),volume rendering( VR) provided by syngo Multi Modality workstation,diagnostic accuracies of the four algorithms on occult fracture were analyzed and compared. Results VR phase had detected 87 occult fractures,total detection rate was 91. 6%; MIP phase had detected 94,the total detection rate was98. 9%; MPR phase had detected 87,the detection rate was 91. 6%; SSD phase had detected 80,the detection rate of 84. 2%,among four groups were statistically significant( P 0. 05). Total detection rate of MIP phase was significantly higher than the other three phases( P 0. 05). Conclusion For patients who are not diagnosed of fractures by X-ray,MSCT examination can be applied,and combined with powerful image post-processing functions,which can significantly improve the diagnostic accuracy of occult fractures,so as to provide reference for the selection of reasonable treatment method.
