NBNA对早产儿早期神经行为异常的影响分析

摘　　要：目的研究新生儿行为神经评分法(NBNA)发现早产儿早期神经行为异常情况及影响。方法选择2010年3月至2013年5月在河北大学附属医院儿科出生的早产儿100例,于纠正胎龄第3、7、28日以及出生后第3日进行新生儿行为神经测定。结果出生后第3日NBNA评分中的行为能力(11.3±1.5)分、被动肌张力(7.9±1.2)分、主动肌张力(7.8±1.3)分、原始反射(5.9±1.3)分、一般反应(6.2±1.9)分以及总分高于出生时的(9.3±1.2)分、(6.4±1.0)分、(6.9±0.9)分、(4.6±1.1)分、(5.5±1.8)分,差异有统计学意义(P<0.01);100例早产儿纠正胎龄后第3日、第7日、第28日NBNA异常率分别为21.0%、17.0%、11.0%,比较差异无统计学意义(P>0.05)。结论NBNA在早产儿早期发现中枢神经系统的异常有重大的临床意义,临床医师应给予重视,根据评分进行早期干预,提高患儿的生活质量。Objective To study the impact of neonatal behavioral neurological assessment（NBNA） and its scoring criteria on finding early neurobehavioral abnormalities of premature infants .Methods A total of 100 premature infants born in the Affiliated Hospital of Hebei University from Mar .2010 to May 2013 were selected as research objects,NBNA was conducted in those babies on the 3rd,7th,28th day of corrected ges-tational age and the third day after birth .Results The NBNA results on the third day after birth included capacity score（11.3 ±1.5）,passive muscle tension score（7.9 ±1.2）,active muscle tension score（7.8 ± 1.3）,primitive reflexes score（5.9 ±1.3）,as well as the general reaction score（6.2 ±1.9）,which were higher than those on the birth day respectively [（9.3 ±1.2）score,（6.4 ±1.0）score,（6.9 ±0.9）score, （4.6 ±1.1）score,（5.5 ±1.8）score],the differences were statistically significant（P〈0.01）.The abnor-mal rates on the 3rd,7th,and 28th day of corrected gestational age were 21.0%,17.0%,11.0%,respec-tively,there were no statistically significant differences（P〉0.05）.Conclusion NBNA and its scoring cri-teria play important roles in finding early abnormalities of central nervous system in premature infants ,there-fore,it is important for the clinicians to pay more attention on NBNA ,so as to conduct early intervention ba-sing on such ratings and improve the quality of life of the children.
