组织多普勒在评价亚临床甲减患者心功能变化中的应用价值

摘　　要：目的应用组织多普勒技术探讨亚临床甲减(SH)早期心脏整体及局部心肌功能的变化。方法选取常规超声检测其舒缩功能指标以及应用组织多普勒技术检测左室侧壁、前壁、下壁和室间隔基底段及中间段的心内膜下心肌层,测量以上各点心肌的收缩期峰值速度以及舒张早期运动速度峰值速度。结果与对照组比较,SH组患者左室室间隔、侧壁、下壁以及前壁的基底段在左室二尖瓣环处4个位点的心肌舒张早期速度峰值明显降低(P<0.05或P<0.01)。结论亚临床甲减患者舒张功能出现明显下降,组织多普勒能够发现早期的心功能改变。Objective To explore the changes of cardiac global and regional myocardial function in early subclinical hypothyroidism（SH） patients by tissue Doppler Imaging. Methods To obtain the conventional cardiac systolic and diastolic function indexes, and to obtain the peak systolic velocity and the early diastolic velocity peak velocity of septal basal and middle segments of subendocardial layer of left ventricular lateral wall, anterior wall, inferior wall by tissue Doppler Imaging. Results Compared with the control groupl the diastolic myocardial velocity decreased significantly in the basal segment of left ventricular septal, lateral wall, inferior wall and anterior wall in SH patients（P〈0.05或P〈0.01）. Conclusion The diastolic function decreased in the patients with subclinical hypothyroidism. Tissue Doppler Imaging can find the changes of cardiac function in the early stage.
