糜蛋白酶联合负压引流治疗腹部感染切口的效果观察

摘　　要：目的：探讨糜蛋白酶联合负压引流装置在腹部感染切口治疗中的效果。方法：将2012年1月~2013年12月40例腹部手术后发生切口感染的患者随机等分为观察组和对照组,两组患者切口处理均采用负压封闭引流技术。观察组每日经切口引流管注入糜蛋白酶溶液,夹管30 min后再开放引流,连续4 d;对照组则注入等量生理盐水,观察两组患者换药期间更换引流管的次数及切口愈合时间。结果：观察组更换引流管次数少于对照组,切口愈合时间短于对照组,差异均有统计学差异（P〈0.05）。结论：糜蛋白酶具有较好的酶学清创效果,结合负压引流技术,有助于改善切口引流,促进切口愈合。Objective： To investigate the value of chymotrypsin combined with negative pressure drainage device treatment of abdominal incision infection.Methods： From January 2012 to December 2013,40 cases of abdominal operation incision infection occurred in patients were randomly divided into observation group and control group. Each group of patients were treated with vacuum sealing drainage. The chymotrypsin solution were injectived into incision of observation group from drainage tube for four consecutive days,and after 30 min the drainage were opened; The control group were injected with normal saline.The drainage condition during dressing period,the average number of replaced drainage tubes. The average healing time of wound of two groups were observed. Results： Compared with control group,the drainage tube clogging rate was lower in observation group,also with less replaced drainage tubes and shortened healing time of incision,the difference was statistically significant（ P〈0. 05）. Conclusion： Chymotrypsin has good enzymatic debridement effect,combined with negative pressure drainage device,it can help to improve the drainage of incision,and promote wound healing.
