四物汤对恶性肿瘤放化疗后血虚证患者的影响

摘　　要：目的探讨四物汤对恶性肿瘤放化疗后血虚证患者临床症状、外周血细胞、免疫功能的影响。方法恶性肿瘤行放化疗的患者60例,随机分为中药组(n=40)和对照组(n=20),中药组又根据四物汤配伍随机分为四物汤组(n=20)及四物合剂组(n=20),口服四物汤煎剂,每日2次,饭前0.5 h服用,连服4周;对照组除不给四物汤煎剂外,其余治疗与中药组相同。对3组患者治疗前后中医症状评分、外周血细胞分析及T淋巴细胞亚群进行统计学分析。结果中药组(四物汤组及四物合剂组)治疗后中医症状改善率均明显高于对照组;四物合剂组白细胞、四物汤组血红蛋白均有升高趋势,但差异无统计学意义(P>0.05);四物合剂组CD4+/CD8+比值显著高于对照组(P<0.05),中药组CD3+、CD4+均有升高趋势,CD8+、CD3-/CD19+均有下降趋势,但差异无统计学意义(P>0.05)。结论中药四物汤可明显减轻恶性肿瘤患者放化疗期后血虚证症状,部分改善其免疫功能,但对外周血细胞影响作用不明显,其中四物合剂配伍可改善患者免疫功能。Objective To explore the effects of Si- wu decoction on the peripheral blood cells,clinical symptoms and the immune function on blood- deficient patients with malignant tumor after radiotherapy and chemotherapy. Methods Sixty cases of patients with malignant tumor after radiotherapy and chemotherapy were randomly divided into the traditional Chinese medicine（ TCM） group（ n = 40） and the control group（ n = 20）. The patients in TCM group were subdivided into Si- wu mixture group and Si- wu decoction group,20 cases in each. Patients in the control group accepted supportive treatment,and patients in TCM group were orally taken Si- wu mixture or Si- wu decoction half hour before meal for four weeks,bid. The data of TCM symptom score,T- lymphocyte subsets and peripheral blood cell of three groups were detected and analyzed before and after treatment. Results The improvement rate of the symptoms in two subgroups of TCM group were significantly higher than that in the control group. The number of peripheral blood cells indicated that data of white blood cells of Si- wu mixture group and hemoglobin of Si- wu decoction group presented a rising trend,but there was no significant differences（ P〈 0. 05）. T- lymphocyte subsets detection indicated that CD4＋/ CD8＋ratio in Si- wu mixture group was significantlyhigher than that in the control group,and the CD3＋and CD4＋account in two subgroups showed a rising trend,while CD8＋and CD3-/ CD19＋accounts were on a downward trend,but there was no statistical difference（ P〈 0. 05）.Conclusion Si- wu decoction could significantly relief blood deficiency symptoms in patients with malignant tumor after radiotherapy and chemotherapy,but no obvious effect on peripheral blood cells was found. Si- wu mixture can improve patients＇ immune function.
