米非司酮与左炔诺孕酮对功能失调性子宫出血患者的疗效与安全性评价

摘　　要：目的评价左炔诺孕酮宫内缓释给药与米非司酮治疗围绝经期功能失调性子宫出血患者的临床疗效和安全性。方法 106例围绝经期功能失调性子宫出血患者随机分为试验组和对照组,各53例,试验组患者用左炔诺孕酮缓释系统治疗,对照组患者用米非司酮治疗,观察2组患者治疗前后临床疗效、不良反应发生率及对患者子宫内膜厚度、月经量、血红蛋白浓度的影响。结果试验组有效率(94.34%)显著高于对照组(67.92%,P<0.05);试验组患者不良反应发生率(7.55%)显著低于对照组(33.96%,P<0.05)。治疗后,2组患者子宫内膜厚度、月经量显著降低,血红蛋白浓度显著升高(P<0.05),且试验组治疗后上述指标变化更为显著(P<0.05)。结论左炔诺孕酮宫内缓释系统治疗围绝经期功能失调性子宫出血疗效显著,不良反应发生率低。Objective To evaluate the clinical effect and safety of levonorgestrel releasing intrauterine system and mifepristone on peri-menopausal dysfunctional uterine bleeding patients. Methods One hundred and six patients with peri- menopausal dysfunctional uterine bleeding were divided into experiment group（ n = 53） and control group（ n =53）,and patients in experiment group received levonorgestrel drug delivery system treatment,while patients in the control group were given mifepristone. The data of effect, the adverse reactions, endometrial thickness,volume of menstruation and hemoglobin concentration of the two groups were compared. Results The drug efficiency of experiment group（ 94. 34%） was significantly higher than that of control group（ 67. 92%,P〈 0. 05）,and the incidence of adverse reactions in experiment group（ 7. 55%） was significantly lower than that in control group（ 33. 96%,P〈 0. 05）. The data of endometrial thickness and the amount of menstrual of experiment group were significantly lower than that of control group,hemoglobin concentration was significantly higher than that of control group. Conclusion The levonorgestrel releasing intrauterine system has good effect on peri- menopausal dysfunctional uterine bleeding,with less adverse reactions.
