摘 要:目的探索坐骨神经分支选择性损伤(spared nerve injury,SNI)模型中糖皮质激素受体(glucocorticoid receptor,GR)激动剂地塞米松对大鼠痛敏的影响。方法取成年雄性SD大鼠24只,随机均分为四组:假手术组(Sham组)、SNI+地塞米松组(SD组)、SNI+GR拮抗剂(RU)组(SR组)和SNI+生理盐水组(SNI+NS组,SS组)。用Von Frey纤维丝测定四组大鼠50%缩足阈值(50%mechanical withdrawal threshold,50%MWT)的变化;SD组和SR组均于术后8d鞘内注射地塞米松(2μg,0.05mg/ml,qd)和RU(2μg,0.025mg/ml,Bid),连续注射4d,术后13d取材,并利用免疫荧光染色检测各组脊髓中GR的表达变化情况。结果与SS组比较,SD组大鼠的50%MWT升高,脊髓背角GR表达明显增加(P<0.05或P<0.01)。而SR组和SS组的50%MWT差异无统计学意义;免疫荧光染色显示SR组脊髓背角GR表达明显低于SS组(P<0.05)。结论脊髓GR参与调节SNI所致的神经病理性痛,鞘内注射Dex能缓解SNI大鼠的机械痛敏。Objective To evaluate the effects of glucocorticoid receptor(GR)agonist dexamethasone(Dex)on mechanical pain sensitivity induced by the spared nerve injury(SNI)model rats.Methods Twenty-four male Sprague-Dawley adult rats were randomly assigned to four groups:groups Sham,SD,SR and SS.50% mechanical withdrawal threshold(50%MWT)was tested by Von Frey filament.Dex(2μg,0.05 mg/ml,qd)and GR antagonist RU(2μg,0.025 mg/ml,Bid)were administered intrathecally from the 8th day to the 12 th day after the surgery in groups SD and SR,the changes of the GR expression in the spinal cord were detected at the 13 th day by immunofluorescence staining.Results The 50% MWT of the group SD was significantly higher than that of group SS,and the expression of GR in the dorsal horn was increased in the group SD(P〈0.05 or P〈0.01).There was no significant difference of 50% MWT between the groups SS and SR,but the GR expression in the group SR was lower than that in the group SS(P〈0.05).Conclusion GR in the spinal cord may mediate the neuropathic pain induced by SNI and there is a substantial reduction of mechanical pain sensitivity by intrathecal injection of Dex.
