乳腺良恶性肿块超声造影特征与骨桥蛋白表达

摘　　要：目的探讨乳腺良恶性组织超声造影(CEUS)特征及其与骨桥蛋白表达的相关性。方法对乳腺良恶性肿块组织行术前超声造影检查,进行超声造影特征分析,术后采用S-P免疫组化染色法分析骨桥蛋白(OPN)的表达情况,分析OPN表达与超声造影特征的相关性。结果 100例病灶中,恶性47例,27例为不均匀性增强的血流灌注模式;良性53例,26例为均匀增强模式;在时间强度曲线分析中,恶性组中有39例呈速升缓降型,良性组中有46例呈缓升速降型。OPN表达在恶性组中有30例呈"++"性,良性组中有34例呈"-"性,乳腺肿块的超声造影特征中增强模式的不均匀增强、TIC曲线中的速升缓降型与OPN的高表达相关(P<0.05)。结论骨桥蛋白的表达与乳腺良恶性肿块的超声造影特征之间存在一定关系。Objective To evaluate the correlation between expression of osteopontin（ OPN） and contrast- enhanced ultrasonography（ CEUS） in malignant and benign breast masses. Methods 100 cases of breast lesions were examined with ultrasonography and CEUS before surgery,and analyzed the characteristics of contrast- enhanced sonography. OPN expression was measured by immunohistochemistry after operation. Statistical analysis was performed to explore the correlation between OPN expression and the feature of CEUS. Results Among 47 malignant masses,27 cases patients showed inhomogeneous enhancemen and 26 cases showed homogeneous enhancement among 53 benign masses. There were 39 malignant masses and 46 benign masses with contrast fast- in- slow- out in the time- intensity curve analysis; There were 30 cases showed ＂ ＋ ＋ ＂in breast cancer,and 34 cases showed＂- ＂in benign ones. The inhomogeneous enhancement in the internal appearance of the opacification and the fast-in- slow- out in the time- intensity curve analysis were correlated to a higher expression of OPN（ P 0. 05）.Conclusions The CEUS is helpful in malignant and benign breast masses diagnosis before surgery. The feature of CEUS may be correlated to OPN expression.
