摘 要:目的:研究纳洛酮联合地塞米松对脑缺血再灌注损伤模型小鼠脑组织的保护作用。方法:取小鼠随机分为假手术(0.9%氯化钠注射液)组、模型(0.9%氯化钠注射液)组、纳洛酮(1 mg/kg)组、地塞米松(1 mg/kg)组和联用(纳洛酮1 mg/kg+地塞米松1mg/kg)组,每组10只。后4组采用双侧颈总动脉同时阻断后再灌注法复制脑缺血再灌注损伤小鼠模型,再灌注5 min后各组小鼠ip给予相应药物。再灌注24 h后对各组小鼠卒中指数和神经病学症状进行评分,显微镜观察神经细胞形态学变化,检测小鼠脑组织匀浆中超氧化物歧化酶(SOD)活性、丙二醛(MDA)含量。结果:与假手术组比较,模型组小鼠卒中指数和神经病学症状评分升高,脑组织匀浆中MDA含量增加、SOD活性减弱,差异具有统计学意义(P<0.01);与模型组比较,纳洛酮组、地塞米松组和联用组小鼠卒中指数和神经病学症状评分降低,脑组织匀浆中MDA含量减少、SOD活性增强,差异具有统计学意义(P<0.01或P<0.05);与纳洛酮组或地塞米松组比较,联用组小鼠卒中指数和神经病学症状评分减小,脑组织匀浆中MDA含量减少、SOD活性增强,差异具有统计学意义(P<0.01或P<0.05)。镜下观察,模型组小鼠神经细胞排列松散,形态异常肿大,出现核固缩等病理变化;纳洛酮组小鼠神经元细胞基本正常,但仍有部分细胞出现核固缩;地塞米松组小鼠出现严重的神经细胞凋亡等病理变化;联用组小鼠神经细胞基本正常,只有极少数细胞异常。结论:纳洛酮、地塞米松对脑缺血再灌注损伤模型小鼠的脑组织具有一定的保护作用,且二者具有协同作用。OBJECTIVE:To investigate the protective effects of naloxone combined with dexamethasone on the mice with cerebral ischemia-reperfusion injury. METHODS:All mice were randomly divided into sham-operated group(physiological sodium chloride solution),model group(physiological sodium chloride solution),naloxone group(1 mg/kg),dexamethasone group(1mg/kg) and drug combination group(naloxone 1 mg/kg + dexamethasone 1 mg/kg),with 10 mice in each group. For the above-mentioned groups except the sham-operated group,the bilateral common carotid arteries of mice were clamped at the same time,and then reperfusion method was adopted to establish mouse models with cerebral ischemia-reperfusion injury. The mice in these groups were given corresponding drugs ip after 5 min reperfusion. The stroke indexes and neurological symptoms were evaluated,their neurocytes were observed under the microscope for morphological changes,and the activities of superoxide dismutase(SOD)and the contents of malondialdehyde(MDA)in their brain tissues were detected after 24 h reperfusion. RESULTS:Compared with the sham-operated group,scores of the stroke indexes,scores of neurological symptoms and MDA contents in homogenate of brain tissues in model group were increased and SOD activities were decreased,with significant difference(P〈0.01). Compared with model group,scores of the stroke indexes,scores of neurological symptoms and MDA contents in brain tissues in naloxone group,dexamethasone group and drug combination group were decreased,SOD activities were increased,with significant difference(P〈0.01 or P〈0.05). Compared with naloxone group or dexamethasone group,stroke indexes and neurological symptoms in drug combination group were decreased,meanwhile, MDA contents were decreased, SOD activities were increased in homogenate of brain tissues,with significant difference(P〈0.01 or P〈0.05).
