外周穿刺中心静脉置管术患者血清炎性因子和凝血四项水平分析

摘　　要：目的:分析外周穿刺中心静脉置管术(PICC)患者凝血四项及血清炎性因子水平的检测结果。方法:未采用PICC治疗的37例患者(非肿瘤患者组15例、肿瘤患者组12例、血液病患者组10例),采用PICC治疗的48例患者(非肿瘤患者组16例、肿瘤患者组14例、血液病患者组18例)。分别对六组患者的血清炎性因子水平和凝血四项水平进行检测并分析结果,对血清TNF-α和IL-6的检测采用放射免疫法;酶联免疫吸附试验测定IL-10,全自动血凝仪测定凝血四项水平。结果:非肿瘤患者PICC治疗组的凝血四项水平和未用PICC治疗的患者组比较,差异无统计学意义。PICC治疗的肿瘤、非肿瘤和血液病患者的TNF-α水平均显著高于未用PICC治疗的患者(P<0.05);hs-CRP检测水平PICC治疗的肿瘤、非肿瘤和血液病患者明显高于未用PICC治疗的患者(P<0.05),采用PICC治疗的患者IL-6水平则低于未采用PICC治疗的患者(P>0.05),但差异不明显,IL-10水平组间无明显变化(P>0.05)。结论:外周穿刺中心静脉置管术后的血清炎性因子中的TNF-α水平、hs-CRP水平和IL-6水平的变化可能由静脉中形成的血栓所导致。objective To analyze the result of the levels of inflammatory cytokines,and four coagulation parameters in serum of peripherally inserted central catheter（PICC） patients.Methods Selected patients without tumor,patients with tumor and patients with blood diseases as the research treated in our hospital.Method A total of 37 patients not performed PICC（including 15 patients without tumor,12 patients with tumor and 10 patients with blood diseases） and 48 patients with PICC（including 16 without tumor,14 patients with tumor and 18 with blood diseases） were collected and then detected and analyzed Serum inflammatory cytokines（TNF-a、hs-CRP、IL-6、IL-10） and blood coagulation function（Prothrombin time,activated partial thrombin time live enzymes,thrombin time,fibrinogen）,the levels of TNF-α and IL-6 in serum were determined by radioimmunoassay,the level of IL-10 by ELISA,and with the level of coagulation parameters（prothrombin time,Active part thrombin time,thrombin time and FIBrinogen）by automatic coagulation machine.Results Compared with the control groups,the levels of coagulation parameters of non-tumor PICC group had not changed significantly,the levels of TNF-α of PICC patients with tumor,without tumor,and with blood diseases were higher（P 0.05）,the levels of TNF-α of PICC patients with tumor,without tumor,and with blood diseases were higher（P 0.05） and the levels of IL-6 were lower（P 0.05）,but the levels of IL-10 had no significant difference（P 0.05）.Conclusion TNF-α and hs-CRP become more higher and IL-6 becomes more lower after PICC,suggesting PICC may lead to venous inflammation and then result in thrombosis.
