普通器械经脐单孔腹腔镜胆囊切除术64例分析

摘　　要：目的探讨普通器械经脐单孔腹腔镜胆囊切除术的手术体会及应用前景。方法回顾性分析2013年4月—2014年3月我院64例普通器械经脐单孔腹腔镜胆囊切除术患者的临床资料。结果有1例因胆囊萎缩,解剖胆囊三角困难,于剑突下、右肋缘下锁骨中线下各增加5 mm Trocar 1个,改为三孔法,其余63例均以单孔法顺利完成手术。手术时间35 min^80 min,平均50 min;术中出血5~40 m L,平均10 m L。术后无出血、胆管损伤、胆瘘、切口感染、脐疝等并发症发生。术后住院时间2 d^4 d,腹壁瘢痕隐蔽,美容效果好。结论普通器械经脐单孔腹腔镜胆囊切除术具有安全性及微创性,美容效果更佳,并减轻了患者的经济负担,值得临床推广。Objective To investigate surgery experience and application prospects of the transumbilical single port laparoscopic cholecystectomy（TSPLC）.MethodsThe clinical data of 64 patients who were treated by TSPLC in our the hospital during April 2013 and March 2014 were analyzed retrospectively.Results63 cases were completed by TSPLC except one case which was completed by The traditional three-hole method since it was difficult to separate the gallbladder triangle because of the atrophy of gallbladder. The operation time was 35 ~ 80 min,average of 50 minutes,the amount of bleeding is 5 to 40 mL,average of 10ml . There was no bleeding,bile duct injury,bile leakage,wound infection, umbilical hernia and other complications after surgery,postoperative hospital stay was 2 to 4 days. Abdominal scar is not easy to see and the cosmetic effect was very good.ConclusionTSPLC with the characteristics of both safe and minimally invasive is worthy of promotion since it has a better cosmetic results and could reduce the financial burden on patients.
