改良式包皮环切术与包皮套扎术治疗小儿包皮过长和包茎的疗效比较

摘　　要：目的比较改良式包皮环切术和包皮套扎术治疗小儿包皮过长、包茎的临床疗效。方法按照就诊顺序纳入100例患儿，随机分成改良式包皮环切术组（A组）和包皮套扎术组（B组），对两组术式的手术时间、术中出血、术后6小时疼痛感进行比较，并对术后并发症（出血、感染、包皮嵌顿、包皮粘连）和美容满意度进行随访比较。结果改良式包皮环切术和包皮套扎术的手术时间、术中出血、术后6小时疼痛感差异均较显著（P〈0．05），两种术士式术后并发症和美容满意度差异均无统计学意义（P〉0．05）。结论包皮套扎术具有手术时间短、术中出血少的优点，但术后疼痛感觉较强烈，术后并发症甚至美容随访效果评价方面并无优势，改良式包皮环切术仍具有不可替代的临床应用价值。Objective To compare the clinical value between the modified circumcision and the prepuce ligation for redundant prepuce or phimosis in children. Methods 100 cases were collected according to the visiting sequence, randomly divided into modified circumcision group（group A = 50）and prepuce ligation group（group B = 50）.To compare the operation time, intraoperative bleeding, the degree of pain six hours after the operation. The postoperative complication（bleeding, infection, paraphimosis, adherent prepuce）and cosmetic satisfaction were followed up. Results There were significant statistical difference in two groups in operation time, intraoperative bleeding and the degree of pain（ P 〈 0.05）, but, no significant statistical difference in postoperative complication and cosmetic satisfaction（ P 〉 0.05）. Conclusion Prepuce ligation with advantages of shorter operation time and less bleeding,but the feeling of pain postoperation was stronger.No difference in postoperation complications and cosmetic satisfaction in two groups. Modified circumcision still has an irreplaceable value in clinical application.
