OSAHS伴高血压患者Auto-CPAP治疗前后疗效对比研究

摘　　要：目的探讨应用Auto-CPAP呼吸机通气治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并高血压的临床疗效。方法确诊合并高血压的OSAHS患者50例。均先接受常规抗高血压治疗,因疗效欠佳,继又接受Auto-CPAP呼吸机通气治疗。同时同步进行24 h动态血压及多导睡眠仪监测。对接受Auto-CPAP治疗前后诸项指标进行对比。结果本组患者经Auto-CPAP治疗1周后其呼吸暂停低通气指数(AHI)、爱泼沃斯嗜睡量表(ESS)值、最低动脉血氧饱和度(Sa O2)及觉醒指数与治疗前比较,差异具有统计学意义(P<0.05);血压变化尚不明显。治疗1个月后其醒时血压、AHI、ESS、觉醒指数均较治疗前及治疗1周后明显下降,最低Sa O2明显升高,差异均具有统计学意义(P<0.05)。结论 AutoCPAP通气治疗有助于OSAHS合并高血压患者的血压控制,值得临床推广应用。Objective To investigate the clinical effect of Auto-CPAP respirator ventilation applied in the treatment of obstructive sleep apnea-hypopnea syndrome（OSAHS） complicated with hypertension. Methods There were 50 OSAHS complicated with hypertension patients receiving conventional antihypertension treatment with bad effect. They received Auto-CPAP respirator ventilation for treatment, combined with monitoring of 24 h ambulatory blood pressure and polysomnography. Various indexes were compared before and after Auto-CPAP treatment. Results After 1 week of Auto-CPAP treatment, there were statistically significant differences of apnea hypopnea index（AHI）, Epworth sleepiness scale（ESS）, the lowest arterial oxygen saturation（Sa O2）, and arousal index（P0.05）; however, the changes of blood pressure were not clear. After 1 month of treatment, blood pressure, AHI, ESS, and arousal index were all lower than those in 1 week, and the lowest Sa O2 was increased obviously. The differences had statistical significance（P0.05）. Conclusion AutoCPAP ventilation is helpful for blood pressure control in OSAHS complicated hypertension patients, and it is worth clinical application.
