摘  要:目的探讨荨麻疹患者外周血丙二醛(MDA)、淋巴细胞内DNA损伤及血浆总抗氧化能力(T-AOC)水平与病情的相关性。方法 284例荨麻疹患者,其中急性荨麻疹(acute urticaria,AU)156例,慢性荨麻疹(Chronic urticaria,CU)128例,通过荨麻疹活动性评分系统(UAS)进行病情评分,根据评分将急性和慢性荨麻疹患者病情分为轻度、中度和重度组。收集各组患者外周血,采用ELISA法、单细胞凝胶电泳技术(彗星实验)及化学比色法分别测定外周血MDA、淋巴细胞内DNA损伤及外周血T-AOC水平。以50例健康者为对照组,观察各监测指标变化与荨麻疹病情相关性。结果 (1)氧化损伤水平比较:AU与CU患者中氧化水平均高于正常对照组,但AU和CU病情轻度患者与对照组比较差异无统计学意义(P﹥0.05);两组患者组内比较,随着病情加重,外周血MDA及氧化损伤水平升高(P﹤0.01);两组患者组间比较,在相同病情下AU外周血MDA及氧化损伤水平高于CU患者(P﹤0.01);而随着病情加重氧化损伤水平加重。(2)抗氧化水平比较:与对照组比较,AU及CU患者外周血T-AOC分别低于对照组,差异有统计学意义(均P﹤0.01);在AU与CU组内比较,两组患者随着病情加重,抗氧化水平降低(P﹤0.01);AU与CU组间比较,在相同病情下,AU低于CU患者(P﹤0.01);而随着病情加重,抗氧化水平降低。结论荨麻疹患者机体内存在氧化与抗氧化失衡,氧化损伤程度与患者病程及病情相关。Objective To investigate the expression of MDA, DNA damage and T-AOC in peripheral blood of patients with Urticaria, Methods The severity of 284 patients with uricaria ( 156 patients with acute urticaria, AU, 128 patients with Chronic urticaria, CU) was evaluated by Urticaria Activity Score (UAS), as mild, moder- ate and severe. The expression of MDA, DNA damage and T-AOC in patients with AU or CU and 50 normal controls were measured by ELISA, Comet Assay and Colorimetrie method. Results ( 1 ) Comparison of oxi- dative damage: the expression of MDA and DNA damage were higher than normal control, but there was no statistcal difference between AU and CU in mild group (P 〉 0. 05 ). With exacerbations, elevated levels of oxidative damage and MDA were seen in patient peripheral blood (P 〈 0. 01 ) . Under the same condition, the MDA levels and oxidative damage in peripheral blood of AU patients was higher than CU (P 〈 0.01). (2) Comparison of antioxidant levels : Compared with the control group, the levels of T-AOC in peripheral blood of AU and CU were lower than that of the control group (P 〈 0. 01 ). With exacerbations, reduced antioxidant levels were seen in AU and CU patients (P 〈 0. 01 ). Between AU and CU groups, the antioxi- dant levels wwere higher than AU patients (P 〈 0. 01 ), and with the exacerbations, antioxidant levels were reduced. Conclusion There is an imbalance between oxidant and antioxidant in the body of patients with urticaria. Oxidative damage is associated with the course and severity of the disease.
