摘　　要：患者女,43岁,周身无力1个月余,面部、颈部皮疹伴发热20d。查体:右侧锁骨上窝及右颈部有握雪感,胸部CT示:纵隔气肿。诊断:皮肌炎合并纵隔及皮下气肿。A 43-year-old female had general weakness for over 1 month and rash on her face and neck associated with fever for 20 days. The physical examination presented holding snow feeling on her right supraclavicular fossa and neck. Chest computed tomography （CT） showed that emphysema was discovered in the mediastinal. The diagnosis of dermatomyositis with mediastinal and subcutaneous emphysema was made.
