下肢静脉曲张术后深静脉血栓形成23例临床分析

摘　　要：目的探讨单纯下肢静脉曲张剥脱术后深静脉血栓形成(deep venous thrombosis,DVT)的治疗方法。方法 23例下肢静脉曲张术后DVT患者,其中15例行抗凝治疗(抗凝组),8例行导管直接溶栓治疗(溶栓组);观察2组近、远期治疗效果。结果 2组住院时间均为14d;抗凝组中5例周围型DVT患者出院时血栓完全溶解,6个月复查无静脉瓣膜功能不全,10例中央型或混合型DVT患者出院时5例血栓部分溶解,5例无明显变化,6个月复查均伴有深静脉瓣膜功能不全;溶栓组8例患者出院时血栓均完全溶解,6个月复查时无静脉瓣膜功能不全。结论导管直接溶栓治疗可作为单纯下肢静脉曲张剥脱术后DVT首选治疗方法。Objective To investigate the therapy of deep venous thrombosis（DVT）after vein ligation and stripping for low limb varix. Methods Twenty-three cases of DVT after vein ligation and stripping for low limb varix were retrospectively analyzed,in which 15 underwent anticoagulation therapy（anticoagulant group）,and 8 underwent catheter-directed thrombolysis（CDT）therapy（CDT group）.The short-and long-term therapeutic effects were observed in two groups.Results The hospitalization stay was 14 days in both two groups.In anticoagulant group,the thrombus in5 cases of peripheral type DVT was completely dissolved at discharge with no vein valve insufficiency after 6months,and10 cases of central and mixed type DVT were partially dissolved the thrombus in 5and showed no obvious change at discharge in 5,with venous valve insufficiency in 10 after 6months.In CDT group,all patients were completely dissolved the thrombus at discharge with no vein valve insufficiency after 6months.Conclusion CDT therapy should used be as the first choice for DVT after vein ligation and stripping for low limb varix.
