多排螺旋CT诊断肺韦格肉芽肿价值

摘　　要：目的探讨肺韦格肉芽肿在多排螺旋CT（multi-slice CT,MSCT）上的影像特征。方法回顾性分析12例肺韦格肉芽肿患者的MSCT影像资料。结果 MSCT表现为双肺多发结节影12例;结节直径0.3~3.0cm;多发结节合并肿块4例,肿块形态不规则,直径3.0~6.5cm;结节周围可见短毛刺1例,见＂供养血管征＂1例,2例可见结节钙化,结节或肿块内见不规则空洞;4例胸膜下见楔形实变;4例可见磨玻璃征;3例见支气管管壁增厚并扩张;1例伴肺不张,1例有双侧胸腔积液。结论 MSCT在肺韦格肉芽肿定性诊断中有重要价值。Objective To analyze multi-slice CT（MSCT）charateristics of pulmonary Wegener＇s granulomatosis.Methods The MSCT imaging data of 12 cases of pulmonary Wegener＇s granulomatosis were retrospectively analyzed.Results MSCT showed multiple nodules in two lungs in 12 cases,with the nodule diameter from 0.3to 3.0cm,in which4 cases were complicated with irregular shaped lumps,with the diameter of 3.0to 3.6cm.Short burr sign appeared in 1case,vascular sign in 1,and calcification in 2around the nodules.Irregular cavity was seen in nodules and tumors.Subpleural wedge-shaped consolidation was found in 4,ground-glass sign in 4,thickened bronchial wall thick and expansion in 3,atelectasis in 1and bilateral pleural effusion in 1.Conclusion MSCT has an important value to the diagnosis of pulmonary Wegener＇s granulomatosis.
