标准化屋尘螨特异性免疫治疗对变应性鼻炎患者外周血CD4^+CD25^+CD127^(low/-)调节性T细胞的影响

摘　　要：目的观察屋尘螨性变应性鼻炎(allergic rhinitis,AR)患者在标准化屋尘螨特异性免疫治疗(specific immunotherapy,SIT)前、后外周血CD4+CD25+CD127low/-调节性T细胞(T regular cell,Treg)比率变化。方法 45例屋尘螨性AR患者随机分为治疗组30例和对照组15例,治疗组接受标准化屋尘螨SIT 6个月,对照组15例接受安慰剂治疗6个月,以CD4+、CD25+、CD127low/-作为标志物,应用流式细胞术检测2组治疗前、后外周血CD4+CD25+CD127low/-Treg细胞比例变化,并与20例体检健康者(正常组)进行比较,记录治疗组和对照组治疗前、后视觉模拟量表(visual analogue scale,VAS)评分变化。结果正常组CD4+CD25+CD127low/-Treg细胞比例((5.319±1.034)%)高于治疗组治疗前((3.644±1.444)%)和对照组治疗前(4.739±2.475)%)(P<0.05);治疗组、对照组治疗后CD4+CD25+CD127low/-Treg细胞比例分别为(6.943±1.514)%、(5.576±1.514)%,均较治疗前增高(P<0.05);治疗组治疗后VAS评分(3.050±2.098)低于治疗前(7.263±1.591)(P<0.05),对照组治疗前、后VAS评分(5.827±2.736、4.236±3.057)比较差异均无统计学意义(P>0.05)。结论屋尘螨性AR患者行标准化SIT治疗后外周血中CD4+CD25+CD127low/-Treg细胞增加,患者症状好转。Objective To explore the changes of CD4＋CD25＋CD127^low/-regulatory T cells in peripheral blood of the patients with allergic rhinitis before and after specific immunotherapy（SIT）with standardized house dust mite.Methods Forty-five patients with allergic rhinitis were divided into treatment group（n=30）receiving SIT with standardized house dust mite for 6 months and control group（n=15）receiving placebo therapy for 6 months.The proportions of CD4＋CD25＋CD127^low/-regulatory T cells were detected by flow cytometric analysis before and after treatment in both groups and were compared with another 20 health controls（normal group）.The changes of visual analog scale（VAS）scores were observed before and after treatment in treatment group and control group.Results The proportion of CD4＋CD25＋CD127^low/-regulatory T cells was higher in normal group（（5.319±1.034）%）than that in treatment group（（3.644±1.444）%）and control group（（4.739±2.475）%）before treatment（P〈0.05）,and were higher in treatment and control groups after treatment（（6.943±1.514）%,（5.576±1.514）%）than those before treatment（P〈0.05）.VAS was lower in treatment group after treatment（3.050±2.098）than that before treatment（7.263±1.591）（P〈0.05）,and there was no significant difference in VAS in control group after treatment（4.236±3.057）in comparison with that before treatment（5.827±2.736）（P〉0.05）.Conclusion The proportion of CD4＋CD25＋CD127^low/-regulatory T cells increases significantly after SIT with standardized house dust mite,and the symptoms are improved.
