摘 要:目的:通过观察原发性膀胱颈梗阻的患者膀胱颈组织的肾上腺素能α1-A受体、肾上腺素能α1-D受体及肾上腺素能β3受体的表达情况探讨其在原发性膀胱颈梗阻发生过程中可能的作用和意义。方法:收集原发性膀胱颈梗阻患者的膀胱颈组织标本,为梗阻组,选择同期住院患有膀胱肿瘤并行膀胱部分切除术或根治性膀胱切除术的患者的膀胱颈组织标本,为对照组。采用免疫组织化学ABC法检测肾上腺素能α1-A受体、α1-D受体及β3受体在以上两组间的表达,比较其在不同组中的表达差异。结果:膀胱颈组织中α1-A受体、α1-D受体和β3受体均有表达。梗阻组的α1-A受体和α1-D受体表达(范围和强度)明显高于对照组,梗阻组的β3受体表达范围明显低于对照组,表达强度与对照组比较,差异无统计学意义(P>0.05)。结论:膀胱颈组织存在αl-A、αl-D和β3三种受体。αl-A受体和αl-D在膀胱颈的过度表达和兴奋是引起膀胱颈梗阻患者排尿困难的主要原因,β3受体表达降低对排尿困难的临床意义有待进一步研究。Objective:To elucidate the possible role of adrenergic alpha1-A receptor,adrenergic alpha1-D receptor and adrenergic beta3 receptor in primary bladder neck obstruction through observing bladder neck tissue adrenaline alpha1-A receptor,adrenergic alpha1-D receptor and adrenergic beta3receptor's expression in patients with primary bladder neck obstruction.Methods:We collected bladder neck tissue samples from patients with primary bladder neck obstruction and selected bladder neck tissue specimens from bladder cancer partial cystectomy or radical cystectomy in patients within the same period as a control group.Immunohistochemical ABC method was used to detect adrenergic alpha1-A receptor,adrenergic alpha1-D receptor and adrenergic beta3receptor's expression between the two groups.Results:1.Expressed alphal-A receptor alpha1-D receptor and beta 3receptor were observed in all bladder neck tissues.2.Expression level of alpha1-A receptor,alpha1-D receptor of obstruction group(scope and intensity)was significantly higher than that in the control group,but expression range of beta 3receptor of the obstruction group was significantly lower than that of the control group though there was no significant difference between the two groups(P〉0.05).Conclusions:1.Alphal-1A,alphal-1Dand beta3 receptor can be observed in bladder neck tissue.Over expressed alphal-A receptor and alphal-D receptor in the bladder neck tissue may mainly contribute to the dysuria,but the role of beta 3receptor in the occurence of dysuria needs further study.
