摘  要:目的:探讨损伤控制技术在骨科的应用效果及对血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、磷脂酶A2(PLA2)水平的影响。方法:选取被本院骨科收治的严重骨折合并创伤患者120例为研究对象。将120例患者随机分为观察组和对照组各60例。观察组给予损伤控制技术治疗,对照组给予传统方法治疗。统计受试者入院后3d体温、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)恢复时间及并发症情况;检测受试者入院后1、3、7、10、14d血清IL-6、TNF-α、PLA2含量。结果:观察组PT、APTT、体温恢复时间均少于对照组,差异有统计学意义(P<0.05);观察组入院后3、7、10、14d血清IL-6、TNF-α、PLA2水平明显低于同期对照组,差异具有统计学意义(P<0.01);观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:将损伤控制技术应用于骨科,待炎症反应得到控制,机体生命体征恢复正常后再实施手术,可减少并发症,提高综合疗效,因此具有广泛的应用前景。Objective:To study influence of damage control technology on TNF-α,IL-6,PLA2 levels.Methods:A total of 120 cases of severe fraction and complicated with trauma were selected as the research objects,andwere randomly divided into observation group and control group with 60 cases in each group.Patients in observation group received damage control technique treatment,while patients in control group had traditional treatment.Body temperature 3dafter admission,prothrombin time(PT),activated partial thromboplastin time(APTT)and complications incidence were observed;Serum IL-6,TNF-alpha,PLA2 content 1d,3d,7d,10 d,14dafter admission were tested.Results:PT,APTT,body temperature recovery time of patients in the observation group were significantly lower than those of the control group(P〈0.05).Serum IL-6,TNF-alpha,PLA2 levels of observation group were significantly lower than those of control group3 d,7d,10 d,14dafter admission(P〈0.01).The incidence of complication of observation group was significantly lower than that of the control group(P〈0.05).Conclusion:Control technology can control inflammation.Application of surgery after recovery of body vital signs to normal can reduce the incidence of complications,improve the comprehensive efficacy.And it is of promising application.
