高原环境下血涂片血小板形态变化及其临床意义

摘　　要：目的探讨高原环境下外周血涂片血小板形态学检查的临床应用价值。方法选经临床确诊的原发性血小板减少性紫癜、血栓性血小板减少性紫癜、骨髓增生异常综合症、再生障碍性贫血、急慢性粒细胞白血病、血小板无力症、巨大血小板综合症、特发性血小板增多症等患者按常规采集末梢血涂片、瑞氏染色、油镜10×100倍观察血小板的形态。结果在原发性血小板减少性紫癜、血栓性血小板减少性紫癜、骨髓增生异常综合症、再生障碍性贫血、急慢性粒细胞白血病、血小板无力症、巨大血小板综合症、特发性血小板增多症等患者的外周血涂片中观察到8种异常血小板形态,结合相应疾病进行应用。结论外周血涂片血小板形态学检查对原发性血小板减少性紫癜、血栓性血小板减少性紫癜、骨髓增生异常综合症、再生障碍性贫血、急慢性粒细胞白血病、血小板无力症、巨大血小板综合症、特发性血小板增多症等患者的诊断、治疗、预后评估均具有重要的临床意义。Objectives We assessed clinical practical significance of platelets morphology on blood smear in high altitude.Method collecting peripheral blood smear from patients diagnosed of idiopathic thrombocytopenic purpura、thrombotic thrombocytopenic purpura、 MDS、 Aplastic Anemia、 AML、 CML、 Glanzmnn thrombasthenia、 giant platelet syndrome and essential thrombocythemia, we observed platelets morphology by blood smear, wright＇s staining and oil microscope imaging（10×100）.Result we found out 8 types of abnormol platelets morphology from these patients＇ blood smear and put it into use combined with the corresponding diseases. Conclusion platelets morphology on blood smear has important clinical significance for these diseases＇ diagnosis、treatment、evaluation of prognosis.
