合并糖尿病和血压晨峰的原发性高血压患者昼夜血压变化的研究

摘　　要：为了分析合并糖尿病、血压晨峰的原发性高血压患者昼夜血压变化的特点,选择255例原发性高血压患者按有无血压晨峰及糖尿病分为4组:行动态血压检测,分析四组年龄、性别、体质量指数(BMI)、心率、血压昼夜模式的差异性。结果显示,(1)糖尿病组BMI明显高于非糖尿病组(P<0.01);(2)糖尿病晨峰组24h平均舒张压、白天平均舒张压高于糖尿病非晨峰组(P<0.05);(3)非糖尿病晨峰组平均收缩压、平均动脉压、白天平均收缩压、白天平均脉压、24h收缩期血压负荷、白天收缩期血压负荷、夜间收缩压标准差、夜间收缩压变异系数高于非糖尿病非晨峰组(P<0.05);(4)糖尿病非晨峰组平均收缩压、白天平均收缩压、夜间平均收缩压、夜间平均动脉压、24h收缩期血压负荷高于非糖尿病非晨峰组(P<0.05)。在原发性高血压患者中,糖尿病或血压晨峰单独存在时,主要表现为收缩压的升高,两者同时存在时,在收缩压升高的基础上,舒张压也升高。To observe the characteristics of circadian changes in blood pressure of diabetic essential hypertension of MBPS group by ambulatory blood pressure monitoring （ABPM）, 255 essential hypertensive inpatients were divided into four groups according to the morning blood pressure surge and diabete： cardiovascular risk factors, such as age, gender, body mass index （BMI）, heart rate and blood pressure rhythm. （1） BMI of the diabetic group was higher than that of the non- diabetic group. （2） The 24 hour diastolic blood pressure and day diastolic blood pressure of the diabetic group of MBPS were higher than the diabetic group of non-MBPS （P〈0.05）. （3） The 24 hour systolic blood pressure, 24 hour mean artery pressure, day systolic blood pressure, day mean artery pressure, 24 hour systolic blood pressure load, day systolic blood pressure load standard deviation and variation coefficient of night systolic blood pressure the non-diabetic group of MBPS were higher than the non-diabetic group of non-MBPS （P〈0.05）. （4） The 24 hour systolic blood pressure, day systolic blood pressure, systolic blood pressure load, night systolic blood pressure, night mean artery pressure, systolic blood pressure load of the diabetic group of non-MBPS were higher than the non-diabetic group of non-MBPS （P〈0.05）. Systolic blood pressure increased obviously in diabetic or MBPS patients, however, diastolic blood pressure increased obviously in diabetic patients of MBPS.
