摘 要:目的探讨肝组织内性雄激素受体(AR)m RNA和雌激素受体-α(ERα)m RNA表达水平对慢性乙型肝炎(CHB)肝组织病理学状态的影响。方法收集135例CHB患者,其中HBe Ag阳性患者80例,HBe Ag阴性患者55例。肝组织内AR m RNA和ERαm RNA采用反转录定量PCR检测。结果 HBe Ag阳性患者的平均年龄在肝组织不同炎症分级和纤维化分期之间的差异均具有统计学意义(P均<0.01);肝组织内平均AR m RNA和ERαm RNA含量在肝组织不同炎症分级和纤维化分期之间的差异均有统计学意义(P均<0.01)。HBe Ag阴性患者的平均年龄在肝组织不同炎症分级之间的差异无统计学意义(P均>0.05),在不同纤维化分期之间的差异具有统计学意义(P均<0.01);肝组织内平均AR m RNA和ERαm RNA含量在不同炎症分级和纤维化分期之间的差异均有统计学意义(P均<0.05)。根据有序Logistic回归分析结果,HBe Ag阳性患者,只有肝组织内AR m RNA含量有预测肝组织炎症分级和纤维化分期的意义(P均<0.01);HBe Ag阴性患者,仅肝组织内AR m RNA含量有预测肝组织炎症分级的意义(P均<0.01)。结论肝组织内AR m RNA含量是影响CHB肝组织病理学状态的一个独立因素。Objective To investigate the influences of androgen receptor (AR) and estrogen receptor-α(ERα) on pathological status in liver tissue in patients with chronic hepatitis B (CHB). Methods Total of 135 patients with CHB were enrolled, among which, 80 patients and 55 patients were HBeAg-positive and HBeAg-negative, respectively. AR mRNA and ERαmRNA were assayed by reverse quantitative PCR. Results In HBeAg-positive patients, there were signiifcant differences of the mean ages among different inlfammatory grading and ifbrotic staging in liver tissue (P all〈0.01);there were signiifcant differences in both the mean AR mRNA and ERαmRNA levels in liver tissue among different inlfammatory grading and ifbrotic staging in liver tissue (P all〈0.01). In HBeAg-negative patients, there were no signiifcant differences in the mean ages among different inlfammatory grading (P〉0.05), and there were signiifcant differences in the mean ages among different ifbrotic staging in liver tissue (P all〈0.01);there were signiifcant differences in both the mean AR mRNA and ERα mRNA levels among different inflammatory grading and fibrotic staging in liver tissue (P all〈0.05). According to the ordinal Logistic regression analyses, only AR mRNA level in liver tissue had signiifcance for predicting inlfammatory grading and ifbrotic staging in liver tissue in HBeAg-positive patients (P all〈0.01), and only AR mRNA level in liver tissue had signiifcance for predicting inlfammatory grading in patients with HBeAg-negative (P all〈0.01). Conclusion AR in liver tissue is an independent factor of inlfuencing pathological status in liver tissue in patients with chronic hepatitis B.
